The affidavits were submitted pursuant to FS 553.791 by MTCI Private Provider Services. Revegetation bond, utility disconnect letters, and permit applications were also submitted. After appropriate review, the Building Department issued a building permit (a copy of the permit is attached) on December 7, 2015.
