THIS ADDENDUM (Number 1 to Contract No. 92-2012) is made and entered into this '-7 day of
K , 2013, by and between Richmond, Indiana, a municipal corporation acting through its Board of
Aviation Commissioners, 50 North 5 h Street, Richmond, Indiana, (hereinafter referred to as "City"), and Butler,
Fairman and Seufert, Inc., 8450 Westfield Boulevard, Suite 300, Indianapolis, Indiana, 46240 (hereinafter
referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 92 2012 on August 9, 2012, which
contract is incorporated by reference; and
WHEREAS, City wishes to retain Contractor to provide additional services required to include new
electrical vault for taxiway lighting as part of the scope of Contract No..92-2012.
NOW, THEREFORE, in consideration of the. mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
The additional services and compensation are set forth in the attachment provided by Contractor
and such services shall be performed in a manner consistent with Contract No. 92-2012, which
attachment provided. by Contractor is hereby incorporated by reference, is made a part of: this
Addendum to said Contract, and is attached hereto as Exhibit A.
2. City and Contractor expressly agree that all other terms, conditions, and covenants contained .in
Contract No. 92-2012, shall be applicable to the work performed pursuant to this Addendum,
unless such term, condition or covenant conflicts with this Addendum, in which case this
Addendum shall be controlling.
.3. Both parties agree that in accordance with Indiana Code 5-22-16.5-1 et seq., contracts with
municipal units entered into or renewed after June 30, 2012, must include the Iran Investment
Activities certification, which certification is set forth and incorporated into Contract Number
92-2012 as follows:
SECTION VI, 22, IRAN INVESTMENT ACTIVITIES
Pursuant to Indiana Code (IC) 5-22-16.5, ENGINEER certifies that
ENGINEER is not engaged in investment activities in Iran. In the event
OWNER determines during the course of this Agreement that this certification
is no longer valid, OWNER shall notify ENGINEER in writing of said
determination and shall give ENGINEER ninety (90) days within which to
respond to the written notice. In the event ENGINEER fails to demonstrate to
the OWNER that the ENGINEER has ceased investment activities in Iran
within ninety (90) days after the written notice is given to the ENGINEER, the
OWNER may proceed with any remedies it may have pursuant to IC 5-22-16.5.
In the event the OWNER determines during the course of this Agreement that
this certification is no longer valid and said determination is not refuted by
ENGINEER in the manner set forth in IC 5-22-16.5, the OWNER reserves the
right to consider the ENGINEER to be in breach. of this Agreement and
terminate the agreement upon the expiration of the ninety (90) day period set
forth above.

4. Both parties agree that with the exception of the changes set forth in Exhibit A and the addition
of the Iran investment Activities Certification, Contract No. 92-2012 shall remain intact and
unchanged.
5. Both parties agree that any person executing this Contract in a representative capacity hereby
warrants that he or she has been duly authorized by his or her principal to execute this
Addendum.

AMENDMENT NUMBER 1 TO AGREEMENT BETWEEN OWNER AND ENGINEER FOR
PROFESSIONAL SERVICES
THIS AMENDMENT, made.and entered into this day of
2013, by and between the CITY OF RICHMOND BOARD OF AVIATION COMMISSIONERS
(OWNER) and BUTLER, FAIRMAN and SEUFERT, INC. (ENGINEER) shall be attached to and
made a part of the AGREEMENT made on August 9", 2012, between OWNER and ENGINEER
providing for professional engineering services.
WHEREAS, the OWNER requires to amend the Design Phase and Bidding and .Negotiating
Phase of the Basic Services to include additional professional engineering services in connection
with the Taxiway "A" Lighting Design (hereinafter called the PROJECT).
WHEREAS, the PROJECT requires additional design services to include an electrical vault;
WHEREAS, the OWNER desires the preparation of bidding and construction documents for the
purposes of including an electrical vault to the PROJECT; and
WHEREAS, the OWNER wishes to engage the ENGINEER to provide certain services
pertaining thereto.
NOW THEREFORE, pertaining to the required basic services for the PROJECT,. the OWNER
and ENGINEER in consideration of their mutual covenants and purposes of completing the
PROJECT herein agree to amend or supplement the AGREEMENT as set forth below.
UNDER APPENDIX "A "SERVICE OF ENGINEER"
Under the Airport Design Phase replace paragraph 4.1.h with the following:
h. Design lighting and signage system, including a new standalone electrical vault, per
FAA standards indentified in various Advisory Circulars, in particular:but not limited to .
Advisory .Circular 150/5340-30 "Design. and Installation Details for Visual Aids",
1.50/5345-10, "Specification for Constant Current Regulators" and 150/5340-18,
"Standards for Airport Sign Systems".
UNDER APPENDIX "D", "COMPENSATION"
Under the Amount_ of Payment Section modify the following categories under section 2
with the following:
Increase Study and Report from $7,000.00 to $9,000.00
Increase Specifications from $5,000.00 to $6,000.00
Increase Plan Set Development from $29,750.00:to $33,750.00 f
Under the Amount:of Payment Section modify the following categories under section
4 with the following:
For Travel increase from $374,00 to $674.00
Electrical Sub Increase $5,000.00 to $10,000.00
