This project was approved by Cullen on 7/5/2013. There was some misunderstanding in the scope of the project and a
deck was torn out and then rebuilt. In the permit application, it only asks for the deck to be torn out. A deck was built
and that was made known when an inspection was performed. I contacted the contractor and he stated that was the
plan all along. I'm sending this back to you in the event it needs to be re-evaluated. Whether you need to revisit the site
or you don't, we will need your authorization.

Ordinary renovation and maintenance activities can create dust that contains lead. Contractors who per -Form
renovation, repairs, and painting jobs in pre -1978 housing and child -occupied facilities must, before beginning
work, provide owners, tenants, and child-care facilitie&with a copy of EPA's lead hazard information pamphlet

Contractors must document compliance with this requirement. After April 22, 2010, federal law will require you
to be certified and to use lead -safe work practices. Georgia Environmental Protection Division Lead -Based
