APPROVING REQUEST FOR PUBLIC RECORDS FORM
WHEREAS, pursuant to Section 2 of P.L. 2001, c. 404, amending and
supplementing the Open Public Records Act (N.J.S.A. 47:1A-1 et seq.),
the
Municipal Clerk is designated the Custodian of Government Records for a
municipality; and
WHEREAS, pursuant to Section 6(0 of P.L. 2001, c. 404, the
Custodian shall adopt a form for use by any person who requests access
to a
government record held or controlled by a public agency, such as a
municipality; and
WHEREAS, the Municipal Clerk and Borough Attorney have reviewed
sample Request for Public Records forms and have modified the same for
use
by the Borough.
NOW, THEREFORE, be it resolved by the Mayor and Council of the
Borough of Carteret as follows:
1. That the Request for Public Records form prepared
by the Municipal Clerk in consultation with the
Borough Attorney is hereby approved as the official
public records request form for use by the Borough.
2. That a copy of said public records request form shall
be distributed to all borough boards, commissions,
bureaus, agencies and offices for use in processing
requests for government records received by any
officer, official and/or employee of said municipal
board or entity

and certified as a true copy of the
original on July 19, 2002.
