ADDENDUM #1 TO CONTRACT NUMBER 158-2012
THIS ADDENDUM is made and entered into this }Z"r14 day of
Nmi e m 2013, by and between the City of Richmond, Indiana, a municipal
corporation acting through its Board of Sanitary Commissioners and its Storm Water
Management Board, (hereinafter referred to as the "City"), and Environmental
Laboratories, Inc., (hereinafter referred to as the "Contractor").
WHEREAS, City and Contractor entered into Contract Number 158-2012 on
January 2, 2013 for the performance of various Iaboratory analysis and pick-up services for
City's vvastewater treatment plant laboratory, MS4 program, and pretreatment program, as
specified, for an initial term of one (1) year; and
WHEREAS, such contract contains an option for the City to renew such
Agreement for two additional one-year terms upon the sarne terms as those stated in
Contract Number 158-2012; and
WHEREAS, the City desires to renew and extend such contract for a first
additional year pursuant to such option.
NOW, THEREFORE, in consideration of the mutual promises and covenants
herein contained, including the above stated recitals, City and Contractor hereby agree as
follows:
1. SECTION ITT. TERIM OF AGREEMENT of Contract No. 158-2012 will
be revised as follows:
The term of this Agreement shall continue, without interruption, until
December 31, 2014.

IN WITNESS WHEREOF, the parties execute this Agreernent effective the day and year
first above written, although signatures may be affixed at different tunes.
