e THIS A ENDUM, (Number 2 to Contract No. 8-2012) is made and entered into this
-&-day of V , 2013, by and between Richmond, Indiana, a municipal corporation
acting through its Board of Public Works and Safety with its office at 50 North 5th Street, Richmond,
Indiana, (hereinafter referred to as "City"), and Pedro Feliciano, d/b/a Preventive Maintenance
Services, 129 South 5th Street, Richmond, IN 47374 (hereinafter referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 8-2012 on January 26, 2012,
which contract is incorporated by reference as well as Addendum Number 1
to said Contract entered into between the parties on December 20, 2012
(effective as of January 1, 2013) which Addendum is also incorporated by
reference and made a part hereof; and
WHEREAS, Contract No. 8-2012 contains an option to renew which the City wishes to
exercise for the 2014 calendar year; and
WHEREAS, City wishes to retain Contractor to continue providing services in connection
with the general maintenance and repair of all Richmond Fire Department
apparatus and administrative vehicles for the Richmond Fire Department
during the course of the 2014 calendar year within the scope of Contract No.
8-2012 and at the same hourly rate previously set forth in Addendum Number
1 to said Contract.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein
contained, including the above stated recitals, City and Contractor hereby agree as follows:
I. Contractor shall continue to provide.. services in connection with the general
maintenance and repair of all Richmond Fire Department apparatus and
administrative vehicles for the Richmond Fire Department as identified within
Contract No. 8-2012 for the 2014 calendar year at the rate set forth in Addendum
Number I to said Contract. Such services shall be performed in a manner consistent
with Contract No. 8-2012, which is hereby incorporated by reference and made a
part hereof.
2. City and Contractor expressly agree that all other terms, conditions, and covenants
contained in Contract No. 8-2012,- shall be applicable to the work performed
pursuant to this Addendum, unless such term, condition or covenant conflicts with
this Addendum, in which case this Addendum Number 2 shall be controlling.
Both parties agree that Contract No. 8-2012 and its Addendum Number 1 shall
remain intact and unchanged.

4. Both parties agree that any person executing this Contract in a representative capacity
hereby warrantsthat he or she has been duty authorized by his or her principal to
execute this Addendum.
THIS ADDENDUM is ex sited and approved by the undersigned representatives of the City
and Contractor this/ , day of/MW4,17013, to be effective in accordance with its terms as of
January 1, 2014.
