A RESOLUTION OF THE VILLAGE OF KEY
BISCAYNE, FLORIDA; AUTHORIZING THE
VILLAGE MANAGER TO EXECUTE THE
ATTACHED CHANGE ORDERS NUMBERS 1-4
FOR THE WEST MASHTA BRIDGE
CONSTRUCTION PROJECT; PROVIDING FOR
AN EFFECTIVE DATE.
NOW THEREFORE, BE IT RESOLVED BY THE VILLAGE COUNCIL OF KEY
BISCAYNE, FLORIDA AS FOLLOWS:
Section 1. The Village Manager is authorized to execute the attached change orders 1-4
for the West Mashta Bridge construction project.
Section 2. This resolution shall take the effect immediately upon adoption.
PASSED AND ADOPTED this 13th day of October , 1998.

PURPOSE OF CHANGE ORDER: To review the Contract Tune to reflect the time
required to accomplish all the permitting requirements of Dade County DERM and the
Army Corps of Engineers. Additional time is required to:
1) Obtain approval of Miami Dade County Commission at the earliest,.
available hearing, scheduled for April 21, 1998..
2) Obtain approval of Army Corps of Engineers required for dredging work
associated with construction of temporary roadway.

Dear Mr. Kissinger.
At the pre -proposal conference for this project, held on 7/16/97, it was stated that the
existing Geotechnical information may be inadequate, and that the Village will pay direct
cost for any additional geotechnical investigation that Is related to the design of the bridge,
pending approval of the cost by the Village.
We have reviewed the available geotechnical information, and based on our preliminary
pile length calculations, have determined that additional testing is required. We need a
minimum of one SPT'on either side of the bridge to a depth of 50 feet in order to properly
design the piles.
We have obtained a proposal from Keith & Schnars for this additional testing -- which is •
attached, for your review and approval:

Dear Mr. Kasper.
We received you correspondence of September 11, 1998, concerning the cost
breakdown for the work to be performed in conjunction with FP&:. to relocate the
overhead lines underground.
The cost for furnishing and installing four (4) 6" diameter PVC conduits (complete) as
per FP&L requirements is approved except for the 5% mark up.

We will prepare a change order for a lump sum a` .`
The FP&L portion of the cost breakdown is still being reviewed by the Village.

Relocation of Permanent FP&L
Power Lines
Per your request, please find the following Change Order Proposal breakdown for the work to be
performed in conjunction with FP&L to place all permanent power lines under the road bed of the
bridge structure, instead of existing overhead service.
1. Furnish and install four 6" diameter PVC conduits $ 2,400.00
as per FP&L requirements, a total of 400 feet at
$ 6.00 per foot .
2. Furnish and install 6" PVC couplings and radius $ 640.00
bends as necessary.
3. Provide excavation and backfilling as required to $ 450.00
accommodate FP&L conduit up to new pole locations
as necessary.
Total .$ 3,490.00
Mark-up (15%) $ . 523.50
Total Change Order Request
$ 4,013.50
