A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF LYNWOOD
ACCEPTING THE CDBG AGNES AVENUE STREET, WATER, AND
SEWER IMPROVEMENT PROJECT, PROJECT NO. 4011.68.087 AS
COMPLETE; AUTHORIZING THE FILING OF THE NOTICE OF
COMPLETION WITH THE LOS ANGELES COUNTY RECORDER'S
OFFICE AND THE RELEASE OF THE 5% RETENTION 35 DAYS AFTER
THE ACCEPTANCE.
WHEREAS, the Agnes Avenue Street, Water, and Sewer Improvement Project
consists of partial removal and replacement of concrete sidewalk, curb/gutter, driveways
and curb ramps; removal and reconstruction of portions of street sections exhibiting signs
of base failure; removal and replacement of portions of native soil as needed; grind and
overlay with new asphalt the entire street section; adjustment to grade all storm
drain/sewer/utility maintenance hole covers; adjustment to grade all water valve can and
covers; provision of all traffic striping, markings, curb painting, house addresses and
signage. Project also includes water and sewer repairs consisting of removal and
installation of new water main, resilient-wedge valves, service line, water meters, and
removal/construction of sewer main and sewer laterals; and
WHEREAS, the Project limit is along Agnes Avenue from Bullis Road to Atlantic
Avenue; and
WHEREAS, on May 7, 2020, the Notice Inviting Bids for the project was advertised
in the Daily Journal, Bid America, on the City's web site (PlanetBids); and
WHEREAS, the bid opening took place in the PlanetBids Platform on Monday,
June 8, 2020 at 10:00 A.M. (Pacific Time); and
WHEREAS, on July 7, 2020, per Resolution 2020.089, the City Council awarded
the Project construction to Sequel Contractors, Inc. in the amount of$2,540,432; and
WHEREAS, the total project budget approved was $3,730.639. The funding
sources approved were from CDBG Funds, Measure M Bonds, Water Bonds and Sewer
funds; and
WHEREAS, the construction of the Project started on August 24, 2020. The project
completion was on March 2021; and
WHEREAS, the City Manager approved Fifteen (15) contract change orders for
the total amount of$264,382.97 (See attachment A); and

WHEREAS, staff conducted the final inspection on March 2021 and found that all
work was completed in accordance with the project plans and specifications.
NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF LYNWOOD DOES
HEREBY FIND, PROCLAIM, ORDER, AND RESOLVE AS FOLLOWS:
Section 1. The City Council of the City of Lynwood hereby finds and declares that
the foregoing recitals are true and correct, and incorporates them herein as findings and
as a substantive part of this Resolution.
Section 2. That the City Council of the City of Lynwood accepts the CDBG Agnes
Avenue Street, Water, and Sewer Improvement Project, Project No. 4011.68.087 as
complete in accordance with contract documents and authorizes the Direct of Public
Works or his designee to process the recordation of the Notice of Completion, and
authorizes staff to release the 5% retention 35 days after the Project acceptance.
Section 3. This Resolution shall go into effect immediately upon its adoption.
Section 4. The City Clerk shall certify as to the adoption of this Resolution.
