A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF LYNWOOD
ACCEPTING THE CITYWIDE SLURRY SEAL (PHASE 1) PROJECT FY
2021-2022, PROJECT NO. 4011.68.124, AS COMPLETE; AUTHORIZING
THE FILING OF THE NOTICE OF COMPLETION WITH THE LOS
ANGELES COUNTY RECORDER'S OFFCE AND THE RELEASE OF
THE 5% RETENTION 35 DAYS AFTER THE ACCEPTANCE.
WHEREAS, CITYWIDE SLURRY SEAL (PHASE 1) PROJECT FY 2021-2022
("Project") encompasses the following street segments of 1.) Ernestine Avenue from
Louise Street to Josephine Street 2.) Ernestine Avenue from Josephine Street to Virginia
Avenue 3.) Ernestine Avenue from Virginia Avenue to Agnes Avenue 4.) Ernestine
Avenue from EOP to Louise Street 5.) Josephine Street from State Street to Peach
Street 6.) Josephine Street from Monrovia Avenue to Lindbergh Avenue 7.)
Josephine Street from Lindbergh Avenue to Nevada Avenue 8.) Josephine Street from
Nevada Avenue to Long Beach Boulevard 9.) Josephine Street from Bullis Road to
Ernestine Avenue 10.) Josephine Street from Ernestine Avenue to Gertrude Drive 11.)
Josephine Street from Gertrude Drive to Muriel Drive 12.) Gertrude Drive from Virginia
Avenue to Agnes Street 13.) Gertrude Drive from Cortland Street to Fernwood Avenue
14.) Gertrude Drive from Fernwood Avenue to Louise Street 15.) Gertrude Drive from
Louise Street to Josephine Street 16.) Gertrude Drive from Josephine Street to Virginia
Avenue 17.) Louise Street from Gertrude Drive to Muriel Drive 18.) Lugo Avenue from
Muriel Drive to Harris Avenue 19.) Muriel Drive from Platt Avenue to Walnut Avenue; and
WHEREAS, the Project consisted of placing slurry seal, including pavement cold
milling and overlay and restriping at some locations; and
WHEREAS, on June 21 2022, the City appropriated funding for Citywide Slurry
Seal Project (Phase 1) from Measure M bonds; and
WHEREAS, on June 21, 2022, the City awarded the contract to the lowest
responsible bidder, Doug Martin Contracting Company, Inc. in the amount of $197,136.46
for the project; and
WHEREAS, a budget of $283,877.00 was approved by the City Council in FY 22. The funding
source approved was from Measure M Bonds.; and
WHEREAS, the construction of the Project started on July 25, 2022; and
WHEREAS, there were zero (0) contract change orders; and
WHEREAS, the amount of $173,663.53 was the final construction cost whereas
$3,300.00 was used for the construction inspection services and $16,992.50 for the
project management tasks including the labor compliance, resulting in total cost of this
project at the amount of $193,956.03; and
WHEREAS, the final date of construction was on August 23, 2022 for the Citywide
Slurry Seal (Phase 1) Project FY 2021-2022, Project No. 4011.68.124 and found that all

work was completed in accordance with the project plans and specifications.; and
NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF LYNWOOD DOES
HEREBY FIND, PROCLAIM, ORDER, AND RESOLVE AS FOLLOWS:
Section 1. The City Council of the City of Lynwood herby finds and declares that the
foregoing recitals are true and correct, and incorporates them herein as findings and as
a substantive part of this Resolution.
Section 2. That the City Council of the City of Lynwood accepts The Citywide Slurry
Seal (Phase 1) Project FY 2021-2022, Project No. 4011.68.124 ("Project") as complete in
accordance with contract documents and authorizes the Director of Public Works or his
designee to process the recordation of the Notice of Completion, and authorizes staff to
release the 5% retention 35 days after the Project acceptance.
Section 3. The City Council authorizes the Mayor to execute this Resolution for and on
behalf of the City of Lynwood.
Section 4. This resolution shall take effect immediately upon its adoption.
Section 5. The City Clerkihall certify to the adoption of this Resolution.
