Add 2 layers of 5/8" drywall to the ceiling of elevator #2 Shaft.
In accordance with OBM field report # 3 dated march 27, 2002.
Please see the attached back up and summary for the work in the amount of $ 1,727.00
This credit will be applied against the Owners Contingency.
We respectfully reserve our rights to request additional time should it become apparent
this work will delay the completion of project.

OBSERVATIONS: 1
01. Work continues on the security system, although it I» s been two week ; since a stop was put
on the work by the village manager and subsequently by OBM. In ads iition, the S15,004. +-
approved for the electrified hardware is no longer required. Cummin ;s was told this at the
previous she visit on March 20th.
02. Nothing has been done on the Dias.
03. Stucco work an the building appears close to complete. The work is e t.ceJient. One minor
correction needs to be made at the tower, see attached sketch.
04. Electrical light fixture locations were verified
REQUIRING ACTION/INFORMATION: 1,
01. OBM discussed cabinet shop drawings and additional costs with cabin .t manufacturer.
The additional costs were eliminated and a small credit is expected.
02. The underside of the elevator shaft roof shall be i hr fire rated. Install 12) layers of Nil"
type 'X' gypsum board.
03. The walls in the day room shall receive openings. The openings in the 'alts shall have a sill
ht. of 42" and shall commence at the columns and ran to ether aide to 1 'Le electrical lines
running in the wall. Both openings shall be the same width. There she uld be no bender and
the ceiling should run continuous from the Day Room to the adjacent (:orridor. If a header
is required match the upper header ht at the Kitchen floor.

plenum of rooms 110 and 105.
05. We are awaiting a cut sheet of a new light of the gym. The new fixture is a flour. Cage
fixture.
06. The range will be moved slightly, to be centered, under the hood. The i;hift is 9" to the
right. The cabinet manuf. was going to coordinate this with the contractor
