THIS ADDENDUM is made and entered into this 5th day of _December,
2007, by and between Richmond, Indiana, a municipal corporation acting through its
Board of Public Works and Safety with its office at 50 North 5th Street, Richmond,
Indiana, (hereinafter referred to as "City"), and Hylant Group of Carmel, Indiana
(hereinafter referred to as "Contractor")
WHEREAS, On May 4, 2001, City and Contractor entered into Contract 38-2001; and
WHEREAS, Contract 3 8-2001 was an Agreement for Contractor to provide health
insurance consulting services; and
WHEREAS, the Contract was amended to reflect a renewal option in Contract 168-
2003; and
WHEREAS, in Contract 4-2005, the parties mutually desired to simultaneously amend
and renew the Agreement for calendar years 2005 and 2006; and
WHEREAS, the parties mutually agreed to extend Contract 4-2005 at a price equal or
less than stated in Contract 4-2005, until December 31, 2007 via Contract 100-2006; and
WHEREAS, due to extensive projects pursued during the 2007 calendar year, City and
Contractor desire to continue the services provided until December 31, 2008; and
WHEREAS, City and Contractor desire to extend Contract 4-2005 until December 31,
2008 at a price equal to or less than that stated in Contract 4-2005.
NOW, THEREFORE, in consideration of the mutual promises and covenants
contained herein, including the above -stated recitals, the City of Richmond, Indiana, by
and through its Board of Public Works and Safety and Hylant Group of Carmel, Indiana
hereby agree that the above -referenced Agreement entered into between the City and
Contractor shall be extended through and including December 31, 2008, and that
Contractor shall be paid an annual sum of Twenty Thousand Dollars ($20,000.00) for
complete and satisfactory performance of the work required hereunder. This Agreement
shall not be renewed or extended beyond December 31, 2008.

IN WITNESS WHEREOF, the parties execute this Agreement effective the day and year
first above written, although signatures may be affixed at different times.
