McIntyre St, Suite 210 Key Biscayne, FL 33149 Re: FDEP Local Government Funding Request — Technical Support Services Dear Mr. Gilbert: EAC Consulting, Inc., as part of its Continuing Services Contract with the Village's respectfully submits this services to assist the Village in preparing and submitting a Florida Department of Environmental Protection (FDEP) Local Government Funding Request (LGFR) for the 2019/2020 fiscal year relative to the Village of Key Biscayne Beach Nourishment Project (Project). The scope of services will be provided by our team including our sub consultant - Moffatt & Nichol providing engineering support services. The following work tasks outline the scope of services: Task 1: 2019/2020 Beach Funding Application Our team shall prepare a FDEP LGFR for the 2019/2020 fiscal year for the Project. The Village shall provide information regarding available municipal beach management support staff and a Council Resolution noting support of the Project, willingness to serve as local sponsor, ability to provide the local cost share, and identification of a dedicated funding source. The Village will provide our team with available data regarding public beach access and parking spaces, location of public restrooms in proximity to the beach, location of public lodging establishments and land use information. Our team shall utilize Project drawings along with permitting, design, construction administration, and monitoring costs associated with the 2017 truck haul nourishment for submittal with the LGFR. Our team shall compile the necessary data and submit the scope of work, schedule, contract copies and other required exhibits to the FDEP Beach Management Assistance Funding Program in support of this funding request. Task 2: Beach Funding Coordination Meetings Our team shall shall attend meetings or participate in telephone conference calls related to the Project as authorized by the Village. Meetings may include attendance at Commission meetings, meetings with Village staff and meetings with state agencies. Approximately twelve (12) hours are budgeted for this effort. If additional meetings are required, these will be provided under an addendum scope of services. Additional Services Services not included in this scope include, but are not limited to the following: 1. Addressing additional data collection/analysis requested by the regulatory agencies 2. Addressing any unforeseen/extenuating circumstances. 3. Additional Meetings unaccounted for in this scope of work. Contract Terms It is our understanding the statement of work will be undertaken under a work order to be issued subject to the terms and conditions of the professional services agreement, dated May 20th, 2016 entered into between the Village and EAC Consulting, Inc. 815 NW 57 Avenue, Suite 402 !Miami, FL 33126 I Phone: 305-264-2557 1 Fax: 305-264-8363 I www.eacconsult.com I CA # 7011 Compensation Compensation for the varying tasks is on a lump sum or hourly basis that is based on the following breakdown and will be invoiced on a percentage basis for lump sum tasks and hours expended for hourly tasks in accordance with the level or progress of completion at the time of billing. Task No. Description Basis Fee 1 2019/2020 Beach Funding Application Lump Sum $ 8,408.00 2 Beach Funding Coordination Meetings Hourly $ 2,839.00 Printing and Reimbursable(s): Lump Sum $ 100.00 Total Compensation for this Work Order: $ 10,897.00 Schedule The schedule for the 2019/2020 fiscal year FDEP funding application consulting services is approximately six (6) months.
