It has come to our attention that there is an Address Numbering Problem in the block in which you
reside. This causes a Public Safety Problem for the 911 Dispatch of Emergency Services (Fire, Police
and Ambulance). Therefore, it is necessary to change / correct your address.
Please be advised effectively immediately your address will be changed:
FROM # 38 B Solomon Avenue
TO # 17 North Campbell Avenue ‐‐‐ New PIN #4‐0002‐16‐020
* No change to 38 Solomon Avenue ‐‐‐ Will keep original PIN#4‐0002‐16‐018*
Please attach the correct House Number on your residence so it can be clearly seen from the North
Campbell side of the house.
* If a Landline Phone is located at this residence, please contact your Phone Company and notify
them of the address change. To confirm the change has been made in the 911 Emergency Dispatch
System call 911 within 7 business days.
We will notify the Tax Assessor, Water Department, Planning & Zoning Department, Fire and Police
911 Dispatch.
Please notify your Insurance Company, Phone Company and any other agencies you deem
necessary. We regret any inconvenience this change may cause you but we know you will recognize
the importance of the change / correction.
