This is to confirm that at the Public Hearing held by the Archit-
ectural Review and Planning Board on September 11, 2003, your
application to permit construction of an addition of 1,340 square
feet, consisting of a one story great room and master bedroom was
considered.
The Level 2 Architectural /Site Plan was approved as submitted with
the following conditions: 1) Palm Beach County Health Department
approval of the septic system shall be submitted prior to issuance
of a building permit; 2) The existing storage shed shall be
removed prior to issuance of a Certificate of Occupancy.
This approval shall expire on September 11, 2004 if a Building
Permit Application, including the aforementioned improvements,
has not been applied for.

1. PBC Health Department approval of the septic system shall be submitted prior to issuance of
a building permit.
2. Existing storage shed shall be removed prior to C /O.
