APPROVING LEAVE OF ABSENCE FOR LISANDRO E. VILLALBA
WHEREAS, Lisandro E. Villalba has requested a Leave of Absence from his
position of Truck Driver; and
WHEREAS, the Mayor and Council has agreed to grant him a Leave of Absence,
for a period of six (6) months; and
NOW, THEREFORE, BE IT RESOLVED that a Leave of Absence is hereby
granted to:
LISANDRO E. VILLALBA
upon the conditions specified by the Mayor on the attached letter, commencing
April 1, 2004 and ending October l, 2004.
Adopted this 25th day of March, 2004
and certified as a true copy of the
original on March 26, 2004.

I am writing this letter to request a leave of Jo cc
Absence of about six months, hopefully beginning april 1st or anytime in april. In cos
doing so, I would like to know that I would be allowed to come back to my normal
position. My understanding is that my personal time will be pro-rated to fit the
time I am absent. 1 also would like to know how or ifI can keep my medical coverage
during this period.
This request is just a pure attempt at getting a small business offthe ground for my-
self.
