THIS ADDEN j M, (Number 1 to Contract No. 177-2013) is made and entered into
this day of h , 2014, by and between Richmond, Indiana, a municipal
corporation acting through its Board of Public Works and Safety with its office at 50 North 51h
Street, Richmond, Indiana, (hereinafter referred to as "City"), and Maze Design, Inc., 124 S. 8°i
Street, Richmond, Indiana, 47374 (hereinafter referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 177-2013 on December 19, 2013,
for certain professional services in connection with construction management of
the rehabilitation of property owned by the City of Richmond and located at
1400-1500 Industrial Road, Richmond, Indiana; and
WHEREAS, City desires that Contractor also provide additional professional services of in-
house construction administration and design services as set forth within the
scope of Contract No. 177-2013.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No, 177-2013, on
December 19, 2013. Pursuant to Section XI. (Miscellaneous) of said Agreement,
the Agreement may be amended in whole or in part at any time by filing with the
Agreement a written instrument setting forth such changes singed by both parties.
2. Contractor shall perform additional professional services of in-house construction
administration services and design services in connection with the scope of
Contract No. 177-2013, as set forth in Contractor's proposal dated February 12,
2014, which quote is attached to this Addendum No. 1 as Exhibit A and
incorporated by reference. Contractor agrees to abide by the same. Contractor
shall be paid the additional amount of Thirty-nine Thousand Five Hundred
Dollars and Zero Cents ($39,500.00) which shall result in a total Agreement sum
not to exceed Sixty-nine Thousand Five Hundred Dollars and Zero Cents
($69,500.00) for completion of the work under Contract No. 177-2013 and this
Addendum No. 1 to said Agreement.
3. City and Contractor expressly agree that all other terms, conditions, and
covenants contained in Contract No. 177-2013, shall be applicable to the work
performed pursuant to this Addendum, unless such term, condition or covenant
conflicts with this Addendum Number 1, in which case this Addendum Number 1
shall be controlling.

4. Both parties hereby agree that with the exception of the addition of the above -
referenced professional services to be performed in accordance with Contract
No. 177-2013, Contract No. 177-2013 shall remain intact and unchanged and in
full force and effect.
5. Both parties agree that any person executing this Contract in a representative
capacity hereby warrants that he or she has been duly authorized by his or her
principal to execute this Addendum.

The following are add items as discussed at our project meeting on 2/12/2014 at your office with
Scott French of Wolverine Worldwide. Wolverine has asked that the below professional services be
completed by us to help expedite the project.
ENGINEER'S SERVICES
In -House Construction Administration Services
After the contracts are awarded, we will check all shop drawings and equipment data pertaining
to the project, but only for the limited purpose of checking for conformance with information given
and the design concept expressed in the Contract Documents. Review of such submittals is not
conducted for the purpose of determining the accuracy and completeness of details such as
dimensions and quantities which remains the responsibility of the Contractor.
1 Preparation of change order bulletins. Where change order bulletins are due to
unforeseen field conditions or Owner requested changes. If changes require Engineering
services then we shall provide a fixed fee for such Engineering for Owners approval prior to us
starting work.
2 Response to request for information (RFI's) from the Installing Contractor. We shall work
with owners representative to respond to this in a timely manor to not slow the project
construction time line.

1. Maze design will prepare required plans, specifications and detail to obtain cost for the
following items to the project that were unforeseen during the original bid.
• New roof system for the building.
• New asphalt overlay repair for the existing paring lot..
• Window replacement for the building.
• Communication Wiring
+ Asbestos Survey
+ Printing of Plans and specifications
• Prepare AIA contract
BASIS OF COMPENSATION
Stipulated Sum ("flat fee") of Thirty Nine Thousand and Five Hundred Dollars and No Cents
($39,500.00)
We ask that the original P.O. S055632 in the amount of $30,000 be modified to the new amount of
($69,500.00)
