COMMENCEMENT OF CASE. A petition for liquidation under chapter 7 of the Bankruptcy Code has been filed in this court by or against the person
or persons named above as the debtor, and an order tor relief has been entered. You will not receive notice of all documents filed in this case. All
documents filed with the court, including lists of the debtor's property, debts, and property claimed as exempt are available for inspection at the office of
the clerk of the bankruptcy court.
CREDITORS MAY NOT TAKE CERTAIN ACTIONS. A creditor is anyone to whom the debtor owes money or property. Under the Bankruptcy
Code, the debtor is granted certain protection against creditors. Common examples of prohibited actions by creditors are contacting the debtor to demand
repayment, taking action against the debtor to collect money owed to creditors or to take property of the debtor, and starting or continuing foreclosure
actions, repossessions, or wage deductions. If unauthorized actions are taken by a creditor against a debtor, the court may penalize that creditor. A
creditor who is considering taking action against the debtor or the property of the debtor should review Sec. 362 of the Bankruptcy Code and may wish to
seek legal advice. The staff of the· clerk of the bankruptcy court is not permitted to give legal advice.
MEETING OF CREDITORS. The debtor (both husband and wife in a joint case) is required to appear at the meeting of creditors on the date and at the
place set forth above for the purpose of being examined under oath. Attendance· by creditors at the 'meeting is welcomed, but not required . At the
meeting, the creditors may elect a trustee other than the one named above, elect a committee of creditors, examine the debtor, and transact such other
business as may properly come before the meeting. The meeting may be continued or adjourned from time to time by notice at the meeting, without
further written notice to creditors.
LIQUIDATION OF THE DEBTOR'S PROPERTY. The trustee will collect the debtor's property and tum any that is not exempt into money. At this
time, however, it appears from the schedules of the debtor that there are no assets from which any distribution can be paid to creditors. If at a later date
it appears that there are assets from which a distribution may be paid, the creditors will be notified and given an opportunity to file claims. All creditors
must notifY the court of any change of address.
EXEMPT PROPERTY. Under state and federal law, the debtor is permitted to keep certain money or property as exempt. If a creditor believes that an
exemption of m9ney or property_ is not authorized by law, the creditor may file an objection. An objection must· be filed not later than 30 days after the
com::lusio~ of the meeting of creditors. · ·
Dl~CH;\RGE OF DEBTS. The deqtor is seeking _a discharge of debts. A discharge means that certain debts are made unenforceable against the debtor
persdmilly. ·Creditors \vhose daims against the debtor are discharged may never take action against the debtor to collect the discharged debts. If a creditor
~lieves that the debtor should not receive any discharge of debts under ~ec. 727 of the Bankruptcy Code or that a debt owed to the creditor is not
dtschargeable under Sec. 523(a)(2),(4),(6), or (15) of tl)e Bankruptcy Code, timely at·tion must t>t: taken in the bankruptcy court by the deadline set forth
above labeled "Discharge of Debts." Creditors considering taking :such action may wish to seek legal advice.
DO NOT FILE A PROOF OF CLAIM UNLESS YOU RECEIVE A COURT NOTICE TO DO SO
The trustee may give notice at the meeting of his intent to abandon property unless objections are filed within 15 days.

STATEMENT OF INFORMATION REQUIRED BY 11 USC Sec 341 AND 342
INTRODUCTION
Pursuant to the Bankruptcy Amendments Act of 1984 and
the Bankruptcy Reform Act of 1994, this information sheet
has been prepared to help you understand some of the
possible consequences of filing a bankruptcy petition under
chapter 7 of the Bankruptcy Code. This information is
intended to make you aware of --
(1) the potential consequences of seeking a discharge
in bankruptcy, including the effects on credit
history;
(2) the effect of receiving a discharge of debts;
(3) the effect of reaffirming a debt; and
Reaffirmation agreements are strictly voluntary --they are
not required by the Bankruptcy Code or other state or
federal law. You can voluntarily repay any debt instead of
signing a reaffirmation agreement, but there may be valid
reasons for wanting to a reaffirm a particular debt.
Reaffirmation agreements must not impose an undue burden
on you or your dependents and must be in your best interest.
If you decide to sign a reaffirmation agreement, you may
cancel it at any time before the court issues your discharge
order or within sixty (60) days after the reaffirmation
agreement was filed with the court, whichever is later. If you
reaffirm a debt and fail to make the payments required in
the reaffirmation agreement, the creditor can take action
against you to recover any property that was given as
(4) your ability to file a petition under a different security for the loan and you may remain personally liable

that may affect your situatiOn. Thts mformatton sheet

your specific case, you should consult with your lawyer. to convert your case to a different chapter .
.wHAT IS A DISCHARGE? Chapter 7 is the liquidation chapter of the Bankruptcy Code.
Under chapter 7, a trustee is appointed to collect and sell, if
economically feasible, all property you own that is not
exempt from these actions.
The filing of a chapter 7 petition is designed to result in a
discharge of most of the debts you listed on your bankruptcy
schedules. A discharge is a court order that says you do not
have to repay your debts, but there are a number of
exceptions. Debts which may not be discharged in your
chapter 7 case include, for example, most taxes, child
support, alimony, and student loans; court-ordered fines and
restitution; debts obtained through fraud or deception; and
personal injury debts caused by driving while intoxicated or
taking drugs. Your discharge may be denied entirely if you
for example, destroy or conceal property, destroy, conceal or
falsify records; or make a false oath. Creditors cannot ask
you to pay any debts which have been discharged. You can
only receive a chapter 7 discharge once every six (6) years.
.wHAT ARE THE POTENTIAL EFfECTS OF A
DISCHARGE?
The fact that you filed bankruptcy can appear on your credit
report for as long as 10 years. Thus, filing a bankruptcy
________ p_e_titiQn_may-affect y-our abi_li!Y__to o_!:>tai!! credit in the future.
Also, you may not be excused from repaying any debts that
were not listed on your bankruptcy schedules or that you
incurred after you filed bankruptcy.
.wHAT ARE THE EfFECTS OF REAFFIRMING A
DEBT?
After you file your petition, a creditor may ask you to
reaffirm a certain debt or you may seek to do so on your
own. Reaffirming a debt means that you sign and file with
the court a legally enforceable document, which states that
you promise tcr repay all ,or a portion of the debt that may
otherwise have been discharged · iil your l;Jankruptcy case.
Reaffirmation agreements must-generally be filed with the
court within 60 days after the first meeting of"creditors.
131134192
Chapter 11 is the reorganization chapter most commonly
used by businesses, but it is also available to individuals.
Creditors vote on whether to accept or reject a plan, which
also must be approved by the court. While the debtor
normally remains in control of the assets, the court can
order the appointment of a trustee to take possession and
control of the business.
Chapter 12 offers bankruptcy relief to those who qualify as
family farmers. Family farmers must propose a plan to
repay the creditors over a three-to-five year period and it
must be approved by the court. Plan payments are made
through a Chapter 12 trustee, who also monitors the debtors'
farming operations during the pendency of the plan.
Finally, Chapter 13 generally permits individuals to keep
their property by repaying creditors out of their future
income. , Each chapter 13 debtor writes a plan which must be
approved-by-the-bankruptcy-court.--The-debwr-must--pay-the---
chapter 13 trustee the amounts set forth in their plan.
Debtors receive a discharge after they complete their chapter
13 repayment plan. Chapter 13 is only available to
individuals with regular income whose debts do not exceed
$1,000,000 ($250,000 in unsecured debts and $750,000 in
secured debts).
