JR seconded. 4 yes. 2. J.M. O'Reilly-variance request for 20 Nancy May Path This property is a 3 bedroom cottage that the owner is selling. Septic system is in failure. The following variances are requested: 1. Septic tank not 100' from Coastal Bank 65' held 35' variance requested 2. Septic tank not 10' from water line Water line proposed to be sleeved 3. Pump chamber not 100' from Coastal bank 64' held 36' variance requested 4. Pump chamber not 10' from water line Water line proposed to be sleeved 5. SAS not 100' from Coastal bank 75' held 25' variance requested 6. SAS not 10' from water line Water line proposed to be sleeved Mr. O'Reilly explained why he designed the SAS system in that specific area on the plan. He stated that the grade is the highest at that point and that he wanted to leave room for upgrades of system on adjoining lot if it was necessary. They were due to go before Conservation Commission but the meeting was cancelled. Motion: AP-continue this until the October 21, 2008 meeting so that the Conservation Commission can approve it. DB seconded. 4 yes. 3. J.M. O'Reilly-informal discussion for property on The Channel Way Mr. O'Reilly stated that this property has a septic system that was installed in 1991. The homeowners would like to finish their garage to add living space to the home. Conservation Commission approval would be needed. Property line variances with regards to the septic system would be needed also. Mr. O'Reilly was just looking for the Boards thoughts on this and whether or not they think it would be something they approved. JH stated that she would like the system to be 100' from wetland in order for her to approve it. The rest of the Board did not feel there were any issues with this proposal. 4. Ron Brown-request to keep septic tank in the existing spot-35 Tupelo Drive Motion: Deny the request. AP seconded. 4 yes. W W W.TOWN.BREWSTER.MA.US BOH 10/7/08 Minutes Page 2 5. Thomas & Yvonne Fazio-request to amend disposal Works Construction permit for 113 Evelyn's Drive Attorney, Dwayne Landreth and Mr. & Mrs. Fazio were present. Attorney Landreth spoke about the history of the property. He spoke about approved capacity and how everything in the file but the permit says that this is a 3 bedroom home. NI spoke about the water report (JH asked that she get an interpretation of what it meant). Motion: AP-approve this home to be 3 bedrooms. If any living space is added in the future, it must come before this Board for approval. JR seconded. 3 yes. JH abstained. 6. Robert Patten-52 Turning Mill Road-request to keep SAS less than 300' from pond Motion: DB-grant request. AP seconded. 4 yes. AP-have NI issue a letter that states the system is designed for 5 bedrooms. JR seconded. 4 Yes. Jared Colley-177 Old Chatham Road-request to keep existing septic system for addition Mr. Colley would like to put a bedroom addition on his home. The septic permit states that the system is designed fora 2 bedrooms but the plan and what was installed states 3 bedrooms. Motion: AP-approve request for 3 bedrooms. JR seconded. 4 yes. 8. Donald & Victoria Zerendow-154 Crowell's Bog Road-request to keep SAS less than 300' from pond Motion: AP-approve request. JR seconded. 4 yes. 9. Discuss letter from Zoning Board of Appeals re: White Rock Commons The Board was concerned with this project being so close to Pleasant Bay. They asked that NI convey their concerns to the ZBA and also let them know that they concur with what the Pleasant Bay Alliance had recommended. Bills to be paid: JR-pay bills. AP seconded. 4 yes 1. W.B. Mason folders $30.30 2. VNA August Services $1435.00 3. Nancy Ellis Ice Mileage X81.55 Informational items: 1. Letter from DEP re: Crosby Mansion septic system upgrade 2. Information on the Nickerson Horse Barn from Cape Cod Commission 3. Burial permit information from MAHB 4. Information from State on massage establishments 5. Information on Daniels/Antinarelli ATC Technical Deficiencies response 6. Information from Daniels/Antinarelli water quality monitoring at the wood waste recycling facility 7. Monthly report for Ocean Edge 8. Letter to Jay Merchant re: Cape Sand & Recycling 9. Letter from Bennett Environmental Associates re: Pleasant Bay Health & Living Centers Add on's The Board asked that when there is a 1995 Title V code s~~:,~n ~Ac,~~~at~e deal with any requests for additions, etc. and not bring them before the Board.
