A RESOLUTION: APPROVING ADDENDUM NO.5 TO THE HAZEN AND SAWYER, P.C.
ENGINEERING SERVICES CONTRACT TO PROVIDE CONSTRUCTION
PHASE SERVICES FOR THE ROUTE 643 WATER TANK
WHEREAS, The Route 643 Water Tank is a funded capital improvement project; and
WHEREAS, the scope of work in the Request for Proposals for engineering services for the tank
included providing both design and construction phase services; and
WHEREAS, Resolution No. 2006-34 which awarded the original contract to Hazen and Sawyer,
P.C. included the provision to negotiate construction service fees for each tank project when needed; and
WHEREAS, Addendum No.5 in the amount of $279,497 was negotiated with the firm for
providing the construction phase services; and
WHEREAS, the addendum is within the available budget for the project.
THEREFORE, RESOLVED by the Council of the Town of Leesburg in Virginia that the Town
Manager is authorized to execute Addendum No.5 with Hazen and Sawyer, P.C. in an amount not to
exceed $279,497, paid on actual hours spent for providing the services.
