A RESOLUTION: AWARDING A CONTRACT FOR EMPLOYEE LONG TERM
DISABIL[TY INSURANCE TO UNUM
WHEREAS, the Town of Leesburg provides its employees with a long tens disability
(LTD) insurance; and
WHEREAS, in response to a Request for Proposal for LTD Insurance, the Town received
proposals from the following seven firms:
• American General Life Companies
• Anthem Blue Cross and Blue Shield
• CIGNA Healthcare Mid-Atlantic
• Hartford Accident Insurance Company
• Kaiser Foundation Health Plan of the Mid-Atlantic States, Inc
• Standard Insurance Co (The Standard); and
• Unum
WHEREAS, the proposals were evaluated and shortlisted firms interview by Wells Fazgo
Insurance Consultants along with an employee evaluation team in accordance with the Town's
Purchasing Policy and the Virginia Public Procurement Act; and
WHEREAS, the best of final offer of Unum reduces cost to the Town and increases the
services available to employees.
THEREFORE, RESOLVED by the Council of the Town of Leesburg in Virginia that the
Town Manager is authorized to conclude negotiations and execute athree-yeaz contract, in a form
approved by the Town Attorney, with Unum for long tens disability insuance for [he fiscal yeaz
beginning July 1, 2010 and ending June 3Q, 2013 with an option to extend for up to two additional
one yeaz periods.
