Time and Place of Receipt of Proposals. Proposals for the 2023 Emergency Management Radio Communication Tower, Village of Plainfield, Will County, Illinois, will be received until 10:00 A.M. on April 21, 2023 by email only. All proposals shall be emailed to Zach Zigterman, Commander, at jzigterman@plainfieldpd.com prior to 10:00 A.M. on April 21, 2023 to be considered a valid proposal. Please include “2023 Emergency Management Radio Communication Tower, – Proposal Submittal” in the subject line of the emailed proposal submittal. 2. Description of Work. An approximate statement of work to be completed as follows: Study, survey, design, and erect a 9’ x 80’ self-supporting emergency management radio communication tower at 14805 Wood Farm Road, Plainfield, Illinois (See site plan attached). 3. Information for Proposers. Copies of the Request for Proposal may be obtained, at no cost, from the Village of Plainfield at the Police Department, 14300 S. Coil Plus Dr, Plainfield, IL 60544 or by contacting Village of Plainfield (815) 439-2929 (contact Commander Zach Zigterman at jzigterman@plainfieldpd.com) for information. To be considered for award, qualified turn-key contractor must be licensed to conduct business in Illinois with expertise in all phases of planning, design, and construction of 911 radio communication towers and related components. The Village of Plainfield is exempt from the Illinois State, municipal or county Retailers Occupation Tax, Service Occupation Tax, Use Tax, and Service Use Tax as described in Illinois Compiled Statute Chapter 35. Bid prices shall not include the cost of such taxes. All Proposals must be accompanied by a Bidder’s bond, certified check, bank cashier’s check or bank draft payable to the Village of Plainfield for five percent (5%) of the total amount of the Bid as provided in the Proposer Instructions. The successful bidder will be required to furnish a satisfactory payment bond and performance bond each in the amount of 100% of the contract. 4. Rejection of Proposals. The Board of Trustees reserves the right to reject any or all Proposals and to waive technicalities. There are no assurances that the project will actually be acted upon. Michelle Gibas, Village Clerk END OF ADVERTISEMENT FOR PROPOSALS 3 SPECIAL PROVISIONS 1) SCOPE OF WORK The project location is 14805 Wood Farm Road. Tower will be a self-supporting 9’ x 80’ emergency management radio communication tower. A. Radio Communication Tower a) Illinois SE Stamped drawings will be included. Sabre Model S3TL-X Grade 50 ksi steel, made in the USA. b) Design will be based upon ANSI-TIA-222-G. c) Cable support backets, safety climb, lightning rod, Illinois SE stamped drawings & delivery to be included. B. FCC & FAA Tower certification procurement will be completed by contractor C. Foundation: a) Soil boring, Illinois SE stamped tower foundation design and installation, local on-site soil removal to be included (based on normal soils). D. Tower Erection a) Tower erection completed by hydro crane or gin pole. E. Ice Bridge a) Ice Bridge from tower to building including ports to be included. F. Grounding System a) Grounding system, cadweled ring utilizing #2 tinned solid copper wire- per Motorola R56 specifications G. Site Work a) 20’ x 20’ x7’ tan PVC fencing with one man gate around tower b) Weed fabric, gravel, signage for site 2) Work Hours/Completion Date A. The Contractor shall include date of anticipated completion. B. Per Village of Plainfield Ordinance, the Contractor shall perform all work in the Village of Plainfield as follows: 7:00 A.M. to 7:30 P.M. Monday through Friday. 7:00 A.M. to 5:30 P.M. Saturday No work will be permitted on Sundays or holidays or at other times outside the above working hours without written permission of the Owner. 3) Contact Person The contractor shall provide the owner’s authorized representative with a phone number where a message can be left for the contractor 24 hours a day. An answering machine connected to the contractor’s normal phone line is an acceptable method of meeting this requirement. The contractor shall check for messages left at this phone number every 24 hours or less. 4) Additional Work All requests for work that will require additional payment must be made in writing. Do not start work until the request for additional work has been approved in writing by the owner’s authorized representative. Failure to obtain advance approval before starting the 4 work may result in non-payment or delays in processing payment. Emergency repairs may be made without authorization when immediate repairs are necessary to protect the health, safety, or welfare of the public or when immediate repairs are needed to prevent further property damage. In these situations, make immediate repairs as needed to protect the public or stabilize the situation and immediately notify the owner’s authorized representative. Additional payment for the work will be negotiated with the contractor by the owner’s authorized representative. Unless otherwise noted, request for work that qualifies for an additional payment must be made within two weeks of time when the need for work becomes apparent. Failure to notify the owner of the need for the work within two weeks of the need for work becoming apparent will result in the requirement that the work be completed at the contractor’s expense. 5) Safety A. No part of this work shall be performed or installed in any location or manner that may endanger the health, safety, or welfare of the public now or in the future. Means, methods, techniques, sequencing, etc., are the sole responsibility of the contractor. B. The contractor agrees that he/she shall assume sole and complete responsibility of his/her work, including safety of all persons and property; and that the contractor shall defend, indemnify, and hold the owner and tenants harmless from any and all liability, real or alleged, in connection with the contractor’s performance of work on this project, excepting for liability arising from the sole negligence of the owner or tenants. 6) Warranty All work and materials shall be warrantied for a minimum of two years from the date of acceptance. Any work proving defective within two years for the date of acceptance shall be redone without additional expense to the Village. Prior to final payment the assigned warranty card shall be provided to the Village. 7) Performance Bond The awarded Contractor will provide a certified check, cashiers check, or performance bond in the amount of 100% of the total bid submitted. Said bond will represent a warranty for performance of the terms outlined in this Request for Proposal. Should this contract be terminated in the event that the Contractor fails to perform as outlined in this bid document said bond would be forfeited. If the bond is in the form of certified check or cashiers check during the length of the contract, the Village will not be required to pay interest on said money to cover any costs. 8) Responsibility for Village/Private Property The contractor will be responsible for any and all damage incurred by the contractor. 5 9) List of References The Contractor shall provide to the Village at least three references including contact person with telephone number and address. 10) Selection Criteria The submitted proposals will be reviewed/selected based upon factors in this RFP and including the following 1. Specific design and construction experience in the development of radio communication towers/sites and complexes (5 years experience minimum). 2. Installations in Illinois. Please provide a list of similar work. 3. Project concept/work plan completeness, clarity. 4. Cost. 5. Geographic breadth of service/specific knowledge. 6. Schedule and availability to promptly complete all work. If the Village determines, at our sole discretion, that the Proposer is not qualified, this proposal will be rejected. 11) Insurance Requirements A. Safety. In accordance with generally accepted construction practices, the Contractor will be solely and completely responsible for conditions for the job site, including safety of all persons and property during performance of the work. This requirement will apply continuously and not limited to normal working hours. B. Insurance. Within fifteen (15) days of receiving a Notice of Award, the contractor shall show evidence to the Village that he has complete insurance coverage as required and described in the following paragraphs of these specifications. A Notice to Proceed will not be issued until these requirements are met. The types of insurance the Contractor is required to obtain and maintain for the full period of the Contract will be Worker’s Compensation Insurance, Comprehensive General Liability Insurance, and Builder’s Risk “All Risk” insurance, including indemnity of the Owner as detailed in the following portions of these specifications. Nothing contained in these insurance requirements is to be construed as limiting the extent of the Contractor’s responsibility for payment of damages resulting from his operations under this Contract. As evidence of specified insurance coverage, the Owner may, in lieu of actual policies, accept certificates issued by the insurance carrier showing such policies in force for the 6 specified period. Each policy or certificate will bear an endorsement of statement waiving right of cancellation or reduction in coverage without ten (10) days notice in writing to be delivered by registered mail to the Owner. C. Workers Compensation Insurance. Coverage, to the statutory limitations required by the State of Illinois, shall be maintained for all persons employed, or may be employed, by the Contractor to carry out the work for the duration of the project. D. Comprehensive General Liability Insurance Policy. This may be combined single limit coverage for bodily injury and property damage in the amount of two million dollars ($2,000,000). The policy shall be broad from including coverage for injury, sickness, disease, and death and damage to or destruction of the property of others arising directly or indirectly out of or in connection with the performance of work under this contract. It shall include protection against claims due to explosion, collapse, or underground damage. This policy shall also include as additional named insured, the Owner; including his consultants; and including each of their officers, employees, and agents. It shall be sufficiently broad to insure the provisions entitled “Indemnification”, As discussed in paragraph “E” of this section. E. Indemnification. The Contractor shall defend, indemnify, keep and save harmless the Owner and their respective board members, representatives, agents and employees, in both individual and official capacities against all suits, claims damages, losses and expenses, including attorney’s fees, caused by, growing out of, or incidental to, the performance of the work under the contract by the Contractor of his subcontractors to the full extent as allowed by the laws of the State of Illinois and not beyond any extent which would render these provisions void or unenforceable. To cover the requirement of naming the Owner and each of their officers, employees and agents as additional named insured on the policies described in Paragraphs D and E above. 7 12) Prevailing Wage A. The undersigned shall comply with the applicable requirements of the “Illinois Prevailing Wage Act” if the work is considered a “public work” project as defined by the State of Illinois B. If this contract calls for the construction of a "public work” within the meaning of the Illinois Prevailing Wage Act, 820 ILCS 130/.0let seq. ("the Act"), the Act requires contractors and subcontractors to pay laborers, workers, and mechanics performing services on public works projects no less than the "prevailing rate of wages'' (hourly cash wages plus fringe benefits) in the county where the work is performed. For information regarding current prevailing wage rates, please refer to the Illinois Department of Labor's website. All contractors and subcontractors rendering services under this contract must comply with all requirements of the Act, including but not limited to, all wage, notice and record keeping duties. Contractor shall submit to the Village monthly all certified payroll records for prevailing wage work performed by contractor employees and subcontractors. The certified payroll records must include the following for each employee employed on the project: Name, Address, Telephone Number, Social Security Number, Job Classification, hourly wages paid in each pay period, number of hours worked each day and starting and ending time of work each day. The contractor shall submit these records with a signed statement that the records are true and accurate, that the wages paid to each worker are not less than the prevailing rate and that the contractor is aware that filing records known to be false is a Class B misdemeanor offense. 8 PROPOSAL TO THE OWNER, The Village of Plainfield, Illinois 1. Proposal of (Name and Address of Proposer) for the WORK titled “2023 Emergency Management Radio Communication Tower, Contract”. 2. The undersigned declares that he has carefully examined the Proposal, Specifications, and Special Provisions (if any), and that he has inspected in detail the site of the proposed WORK, and that he has familiarized himself with all of the local conditions affecting the CONTRACT and understands that in making this Proposal, he waives all right to plead any misunderstanding regarding the same. 3. The undersigned further understands and agrees that if this Proposal is accepted he is to furnish and provide all necessary machinery, tools, apparatus to do all of the WORK, and to furnish all of the materials specified in the CONTRACT, except such materials as are to be furnished by the OWNER, in the manner and at the time therein prescribed, and in accordance with the requirements therein set forth. 4. The undersigned further agrees that his proposal will remain subject to acceptance for sixty (60) days after the day of the receipt of Proposals, but Owner may, in its sole discretion, release any Proposal and return the Bid security prior to that date. 5. The undersigned further agrees that this proposal shall serve as the CONTRACT for this WORK if accepted by the OWNER. 6. The undersigned has included a Bidder’s bond, certified check, bank cashier’s check or bank draft payable to the Village of Plainfield for five percent (5%) of the total amount of the Bid. 7. The undersigned further agrees to provide a bank draft, bank cashiers check, certified check, or performance bond for 100% of the total contract price, complying with the requirements of the specifications, made payable to the VILLAGE OF PLAINFIELD if awarded the contract. 8.
