HomeMy Public PortalAbout02465 O R D I N A N C E NO. 2465 AN ORDINANCE, amending Ordinance No. 909, as adopted January 11, 1963, by repealing Sections One, Three and Five and enacting new Sections One, Three and Five in lieu thereof establishing a revised schedule of benefits and payroll deductions therefor. BE IT ORDAINED BY THE BOARD OF TRUSTEES OF THE METROPOLITAN ST. LOUIS SEWER DISTRICT: Section One. Ordinance No. 909, as adopted January 11, 1963, is hereby am ended by repealing Sections One, Three and Five thereof. Section Two. Ordinance No. 909 is hereby further amended by enacting new Sections One, Three and Five thereof to establish a revised schedule of benefits and payroll deductions therefor. Such new sections shall read as follows: "Section One. The District hereby extends to all officers and employees as defined in Ordinance No. 909, adopted January 11, 1963, the benefits of participating group insurance as compensation for services in addition to salaries, compensation and benefits otherwise provided by Ordinance. The net premium cost to the District shall not exceed eighty percent (80%) of the aggregate of the individual premiums for employee medical insurance and employee life insurance of participating officers and employees." "Section Three. Except for such reduction in benefits at age 65 as may be customary and necessary, the benefits hereunder shall be as follows: 3 A. Officer or Employee Class Life Insurance Annual Earnings Amount Less than $ 5,000.00 $ 5,000.00 $ 5,001.00 - 6,000.00 6,000.00 6,001.00 - 7,000.00 7,000.00 7,001.00 - 8,000.00 8,000.00 8,001.00 - 9,000.00 9,000.00 9,001.00 - 10,000.00 10,000.00 10,001.00 - 11,000.00 11,000.00 11,001.00 - 12,000.00 12,000.00 12,001.00 - 13,000.00 13,000.00 13,001.00 - 14,000.00 14,000.00 14,001.00 - 15,000.00 15,000.00 15,001.00 - 16,000.00 16,000.00 16,001.00 - 17,000.00 17,000.00 17,001.00 - 18,000.00 18,000.00 18,001.00 - 19,000.00 19,000.00 19,001.00 - and above 20,000.00 B. Accidental death and dismemberment insurance shall be an amount equal to the life insurance, (A. above). C. For various types of medical care, the maximum amounts shall be as follows: hospitalization room and board $26.00 per day, $1,820.00 maximum per disability; miscellaneous hospitalization charges maximum $300.00 per disability; ambulance benefit $30.00; surgical $300.00 maximum schedule; maternity, hospital $150.00, obstetrical-normal delivery $75.00, caesarean section $150.00, miscarriage $37.50; in-hospital doctor visits $4.00 per call, maximum $280.00; diagnostic X-ray and laboratory $50.00 maximum in any twelve-month period (non-scheduled); maximum payment for emergency accident $100.00; major medical $10,000.00 maximum with automatic reinstatement of $1,000.00 annually. Group coverage hereunder shall be convertible into an individual insurance policy on termination of employment on such terms as are provided in the Group Insurance Contract." 4 "Section Five. There shall be deducted from the earnings of each participating officer and employee twenty per cent (20%) of the premiums for his individual employee medical insurance coverage and individual life insurance coverage. Such deduction shall ordinarily be made in equal installments from earnings for the first and second pay periods ending within each calendar month. 5 The foregoing Ordinance was adopted August 10, 1973.
