WHEREAS, Ordinance No. 12555, adopted December 13, 2007, authorized the
Executive Director and Secretary-Treasurer on behalf of The Metropolitan St. Louis Sewer
District to enter into a contract with Fort Dearborn Life Insurance Company, a wholly-owned
subsidiary of Health Care Services Corporation, for the District’s Employee Group Life and
Accidental Death & Dismemberment Insurance, and
WHEREAS, said contract will expire on January 31, 2010, and
WHEREAS, the District desires to exercise its option to renew said contract for
an additional one-year term, commencing on February 1, 2010,
NOW, THEREFORE, BE IT ORDAINED BY THE BOARD OF TRUSTEES OF
THE METROPOLITAN ST. LOUIS SEWER DISTRICT THAT, the Executive Director and
Secretary-Treasurer are hereby authorized on behalf of The Metropolitan St. Louis Sewer
District to exercise the District’s option to renew for an additional one-year term commencing on
February 1, 2010, the contract with Fort Dearborn Life Insurance Company, a wholly-owned
subsidiary of Health Care Services Corporation, whereby Fort Dearborn Life Insurance
Company, a wholly-owned subsidiary of Health Care Services Corporation, would administer
the District’s Employee Group Life and Accidental Death & Dismemberment Insurance, said
contract renewal shall be comprised of such terms and conditions as are approved by the office
of the General Counsel of the District.
The foregoing Resolution was adopted December 10, 2009.
