PRIVATE SANITARY SEWERS OPERATING PERMIT
ANNUAL OPERATING PERMIT
This document, issued under the provisions of Chapter 24, Miami -Dade County Code (Dade County
Pollution Control Ordinance), shall be valid from 01-OCT-2014 through 30-SEP-2015. The above named
permittee is hereby authorized to operate the pollution control facility at the above location which
consists of the following:

This facility is subject to conditions listed below and in the following pages (if any) of this
permit.
SPECIFIC CONDITIONS
1. The collection/transmission facility shall be operated continuously in such a manner that maximum
level of efficiency is maintained at all times and in full compliance with Rule 62-604, Florida
Administrative Code.
2. All privately -owned or operated sanitary sewer collection systems which have sanitary sewer
pumping stations, shall provide each pump with a fully operational and working non-resettable
elapsed time (ET) meter as required by Section 24-42.2(2)(a) of the Miami -Dade County Code.
Monthly totalized ET meter readings of the total number of hours for each pump shall be recorded
and reported as required by Section 24-42.2(1)(f)(i) and entered by the permittee via an Internet
specifically created for this purpose. As of Oct 1, 2012, this facility is required to start
entering the ET readings utilizing the new Web application. The Web address to start entering the
ET readings and/or to obtain the Web application guidelines is

3. The Permittee shall obtain a Domestic Wastewater Collection/Transmission System Construction
permit and notify this Department before making any modifications of the sanitary sewer
collection system that may change the cap.- of the system.

facility. A copy of the approved manual shall be maintained at the facility site. The manual
shall be updated annually whenever the system is modified or enlarged, or as required by new
regulatory actions pursuant to Chapter 24-42.2(5)(b) of the Code. The facility shall be
maintained in accordance with the practices described in the approved Operations and Maintenance
Manual. Logbooks required by the manual shall be maintained at the facility in a location
protected from the weather.
5. The Permittee shall submit an annual report to the Director or his designee which sets forth a
map and a list of all sewer service areas and sewer subsystems, including the total length (in
feet) of gravity sewer lines according to pipe diameter and type of material, and number of
manholes in each service area. This information shall be submitted only if there have been
changes in the service area.
6. The Permittee shall provide a Sanitary Sewer Evaluation Survey (SSES) for this
collection/transmission facility. Such evaluation activities shall be conducted in a manner so
that the total length of the gravity sewer lines and associated manholes in the sanitary sewer
collection system is evaluated once every ten year period, pursuant to Chapter 24-42.2 (1)(a) of
the Miami -Dade County Code. You may contact the PSO Program at (305)372-6600 in order to obtain a
copy of the SSES Guidelines.
7. The Permittee shall create, post, maintain, update and secure the EMERGENCY CONTACT SIGN. You
shall include in the Emergency Contact Sign, the PSO Permit number, your Emergency Maintenance
Contact Person and PERA's 24 hour complaint desk telephone number (305-372-6955) Whereever
possible, the sign shall be posted on a flat surface and visible at and around the pump station.
In any case, the sign shall be within twenty (20) feet of the wet well of the station, three(3)
feet to six (6) feet above ground level and clear of immediate obstructions. The Permittee shall
update the contact number displayed on the sign, as required, with a durable marker.
8. No additional sanitary sewer connection(s) shall be made to the existing private sanitary sewer
collection system from other facilities or properties without a review and prior written approval
by the PERA.
GENERAL CONDITIONS
9. The applicant, by acceptance of this document, agrees to operate and maintain the subject
operation so as to comply with the requirements of Chapter 24 of the Code of Miami -Dade County.
10. If for any reason, the applicant does not comply with or will be unable to comply with any
condition or limitation specified on this document the applicant shall immediately notify and
provide the department with the following information: (a) a description of and cause of
non-compliance; and (b) the period of non-compliance including exact dates and times; or, if not
corrected, the anticipated time the non-compliance is expected to continue, and steps taken to
reduce, eliminate, and prevent recurrence of the non-compliance. The applicant shall be
responsible for any and all damages which may result and may be subject to enforcement action by
the department for penalties or revocation of this document.
11. As provided in Section 24-15 of the Code of Miami -Dade County, the prior written approval of the
Department shall be obtained for any alteration to this facility.
12. The issuance of this document does not convey any vested rights or any exclusive privileges. Nor
does it authorize any injury to public or private property or any invasion of personal rights,
nor any infringement of federal, state or local laws or regulations. Nor does it relieve the
applicant from liability for harm or injury to human health or welfare or property.
13. This document is required to be posted in a conspicuous location at the facility site during the
entire period of operation.
14. This document is not transferable. Upon sale or legal transfer of the property or facility
covered by this document, the applicant shall notify the department within thirty(30) days. The
new owner must apply for a permit within thirty (30) days. The applicant shall be liable for any
non-compliance of the source until the transferee applies for and receives a transfer of this
document.
15. The applicant, by acceptance of this document, specifically agrees to allow access to the named
source at reasonable times by department personnel presenting credentials for the purposes of
inspection and testing to determine compliance with this document and department rules.
16. This document does not indicate a waiver of or approval of any other department permit that may
be required for other aspects of this facility
17. This document does not constitute an approval by the Department or certification that the
applicant is in compliance with applicable laws, ordinances, rules or regulations. The applicant
acknowledges that separate enforcement actions may be initiated by the Department and that this
document does not constitute compliance with orders issued in conjunction with enforcement

18. Failure to comply with any condition of this document, or the requirements of Chapter 24, Code of
Miami -Dade County may subject the applicant to the penalty provisions of said Chapter including
civil penalties up to $25,000 per day per offense and/or criminal penalties of $500 per day or
not less than $1000 per day for violations of Section 24-42.4, Sanitary Sewer Discharge
Limitations and Pretreatment Standards and/or sixty (60) days in jail.
