RESOLUTION AUTHORIZING EXECUTION OF VARIOUS
APPLICATION DOCUMENTS FOR THE SUSPECT AREAS SEWER
SEPARATION AND IMPROVEMENTS PROJECT
WHEREAS, in order to proceed with the Suspect Areas Sewer Separation And
Improvements Project, Contract No.00-13, various application documents require
execution by the Mayor on behalf of the Borough of Carteret prior to their
submission to the Middlesex County Utilities Authority and/or the NJDEP Bureau
of Construction & Connections; and
WHEREAS, a Treatment Works Approval Permit Application, Form TWA-I,
requires execution by the Mayor, along with a check in the amount of $3,032.57
made payable to "Treasurer, State of NJ. Environment Services Fund", as
required for the application review fee' and
WHEREAS, a Statement of Consent, Form WQM-003, requires execution by the
Mayor; and
WHEREAS, a Freehold Soil Conservation District, Application for Soil Erosion
and Settlement Control Plan Certification, requires execution by the Mayor along
with a check in the amount of $1,235.00 made payable to "Freehold Soil
Conservation District" for an application, review and inspection fee; and
WHEREAS, a review fee of $25.00, made payable to "MCUA", is required by
the Middlesex County Utilities Authority to review and authorize the Statement of
Consent, Form WQM-003.
NOW,THEREFORE, BE IT RESOLVED by the Borough of Carteret, County
of Middlesex, that the Mayor is hereby authorized to execute the foregoing
application documents on behalf of the Borough of Carteret and the payment of
the foregoing application fees are hereby approved.
