Application for listing on Bridge Inventory System
Dear Mr. Kissinger,
The following information is required by the Florida Department of Transportation
(FDOT) in order to list the West Mashta Drive Bridge on the FDOT Bridge
Inventory System;
• a set of 11"x17" plans for the project
• a bridge information sheet identifying key characteristics of the bridge
• a company business card from C3TS (required for being the project
engineer) and
• a letter requesting a bridge number from the FDOT
These items have all been included with this letter so you can mail them to the
FDOT. The FDOT requires that the request for the bridge number originate from
the owner of the structure. I have attached a sample letter requesting a bridge
number for your use. Please address the package to:
