Board Members present: Mary Chaffee, RN, PhD, Chairman; Joanne Hughes, Vice Chairman; Lemuel
Skidmore, MS, MPH; Annie Sherwood, RN; Pat Ottolini, RN, MPA, CHA
Call to order: Chairman called the meeting to order at 7:00pm.
Others present: Nancy Ellis Ice, MPH, CHO, RS, Health Director; Tammi Mason, Senior Department
Assistant;
1. Moran Engineering Associates-Title 5 variance requests for 293 Bog Pond Road
Discussion: Rick Judd presented this to the Board. The property has had a failed septic inspection and will
be upgrading the system. The following variances are requested:
1. Distance between property line & SAS to be 5'
2. Distance between foundation& SAS to be 11.7' & 17'
3. Distance between catch basin& SAS to be 15'
Rick Judd told the Board that a 40 Mil Poly liner will surround the system.
{ Annie Sherwood questioned the extension of the poly mil liner 25 feet beyond the catch basin.
Joanne Hughes asked if there was a full foundation on the house. Rick Judd stated that there was.
Joanne Hughes asked how the distance between the top of the SAS to the depth of the foundation relates to
shortening the distance to the foundation. Rick Judd stated that the poly mil liner will be going up to the extent
of the property line to the west, then will go south, then east, then return to the property line to the west. On
the other side, the liner will be between the septic tank traveling to the southern part of the septic tank and will
then travel towards bog pond road, will go east and then starts to wrap back up in the direction of utilities.
Motion: Finding that the variances are being mitigated by the engineers suggestions as well as the 40 mil
poly liner, grant the variances as listed on the plan for project#13-318, dated 3/20/14.

2. Bennett Environmental Associates, Inc.-local variance request for 96 Canoe Pond Road
Mary Chaffee stated that she had been a previous client of Bennett Environmental Associates and has filed the
appropriate paperwork and spoken with the State Board of Ethics and this will not be in any way a conflict.

Discussion: Joseph Smith presented this request to the Board.
This is a request for a variance to allow the homeowner to keep the SAS 180' from a pond. There was a septic
inspection that passed except for the distance to the pond. The house is currently a 3 bedroom with a 4 bedroom
septic that was installed in 1973. There is a 13' separation from the bottom of the SAS to groundwater
Motion: In view of the fact that there is a 13' separation from the bottom of the SAS to groundwater
and the septic system is as far away as possible from the pond, grant the variance request for the SAS to
remain 180' from the pond.

3. Chatham Bars Inn-Food Establishment permit application
Mary Chaffee stated that she had been a former client of Coastal Engineering and has filed the appropriate
paperwork and spoken with the State Board of Ethics and this will not in any way be a conflict.
Discussion: Sean Riley, Coastal Engineering and Sarah Cole, Chatham Bars Inn presented this request to the
Board.
Mr. Riley stated that there will be a Certified Food Manager on site every day,there will be no foods prepared
on the property.
Joanne Hughes gave Mr. Riley a copy of the Assistant Health Director's recommendations.
It was noted that any changes to the operation would have to come back before the Board of Health for
approval.
Motion: Grant Food Establishment permit subject to the recommendations being met.

4. Ardeo Captains Café-Mobile Food & Snack Shack food permit application
Discussion: Joe Jamiel, owner of Ardeo presented this request to the Board.
Mr. Jamiel stated that all food will be pre-packaged with the exception of the hot dogs.
Joanne Hughes gave Mr. Jamiel a copy of the Assistant Health Directors recommendations.
Motion: Grant Snack Shack permit subject to the recommendations being met.
Motion: Annie Sherwood Second: Pat Ottolini Vote: 5-0-0 Action: Motion carried

5. Cliff Berner-200 South Pond Drive-request for an extension of Real Estate Transfer Inspection
Discussion: Mr. Berner asked the Board for a 45 day extension on his inspection. Joanne Hughes thought it
would be better to give a 90 day extension in case issues arise.
Motion: Grant Mr. Berner a 90 day extension for his Real Estate Transfer Inspection
Motion: Joanne Hughes Second: Lemuel Skidmore Vote: 5-0-0 Action: Motion carried
6. Brewster Farms-Tobacco Regulation violation
Discussion: Jesse Corn, employee for Brewster Farms was present. The Board explained there would be
a $100 fine assessed.
7. Laura Kelley- "Protect Our Cape Cod Aquifer" —Nstar herbicide spraying
Discussion: Ms. Kelley did not attend. There was no discussion.
8. Discuss Planning Board application for Brewster Pizza House
Discussion: It was noted that the septic system is adequate for what was being proposed.
9. Minutes from 3/18/14 Meeting
Motion: Approve minutes as written
Motion: Annie Sherwood Second: Pat Ottolini Vote: 5-0-0 Action: Motion carried.
10. Discuss meeting dates for July
The Board agreed to change the date of the June meetings to June l 7t" (one meeting only)
The Board agreed to change the July dates to July 8 & 22.
11. Topics the chairman did not anticipate
Lemuel Skidmore spoke about the CWPC meeting and stated that the draft BOH regulation will be ready for
limited distribution by the end of next week. Before the draft goes to the BOH, the CWPC will go over it with
Mark Nelson and give their comments.
Informational items were noted.
