PENN CARE, INC., hereinafter referred to as the "LESSOR"
hereby agrees to lease, on a daily basis, One (1) 2001 Ford E350
Cutaway McCoy Miller Ambulance, Vehicle Identification Number
1 FDWE35F31 HB34168 on September 4, 2014 to RICHMOND FIRE
DEPARTMENT, hereinafter referred to as "LESSEE."
LEASE CHARGES shall be $200.00 per day beginning on
September 4, 2014. Full payment shall be made at the termination of
the lease, or if longer than Thirty (30) days, to be made on a monthly
basis. .
LESSEE agrees to provide insurance coverage on the vehicle,
which includes collision coverage, at a FIVE HUNDRED DOLLAR
($500.00) deductible and comprehensive coverage at FIVE
HUNDRED DOLLARS ($500.00) deductible.
LESSEE is responsible for FIVE HUNDRED THOUSAND
DOLLARS ($500,000.00) liability insurance coverage to cover
personnel, equipment and their vehicle during the time the leased
vehicle is being used by them.
LESSEE.agrees to supply LESSOR with a copy of their insurance
policy. In turn, a copy of the fully executed Lease Agreement
Contract will be forwarded to LESSEE's insurance agent.
LESSEE agrees that, upon termination of the lease, they will
return possession of the vehicle in its present condition, reasonable.
wear and tear accepted.
LEASE AGREEMENT shall be terminated upon the completion of
repairs currently being conducted on Braun Unit #3616 and when
said unit is placed back in service.
LESSEE agrees to pay for any damages that may occur to the
ambulance or the chassis, reasonable wear and tear included.
