AUTHORIZING A PROFESSIONAL SERVICES CONTRACT FOR
CONSTRUCTION MANAGEMENT SERVICES WITH CCMS CORPORATION
, ,
WHEREAS, the Borough of Carteret is in need of specialized construction management
services in connection with the upgrade program for the Memorial Municipal Building;
and
WHEREAS, CCMS, 9 Professional Circle, Suite 204, Colts Neck, NJ 07722, possesses
the requisite experience and expertise to provide said services; and
WHEREAS, CCMS has submitted a proposal to provide said services, to include the
upgrade program for the Memorial Municipal Building at a cost not to exceed
$129,480.00; and
- WHEREAS, the Mayor and the Council have reviewed said proposal; and
WHEREAS, the Chief Financial Officer has certified that funds are or will be available
for said purpose.
NOW, THEREFORE, BE IT RESOLVED, by the Mayor and the Council of the
Borough ofCarteret, County of Middlesex, State of New Jersey, as follows:
I. The proposal from CCMS for the upgrade program for the Memorial Municipal
Building at a total cost not to exceed $129,480.00 is hereby approved;
2. This action is approved as an award of a professional services contract.
3. The copy of this Resolution be forwarded to the Borough's Purchasing
Department and CCMS.

Project Approach
Consolidated Construction Management Services (CCMS) is committed to provide
services to the County of Union to achieve successful completion of this construction
proj ect.
The success of any construction Project, particularly a multi-faceted, and complex project
such as this requires a Construction Manager with the following attributes:
, Properly licensed with the State of New Jersey - Construction Management is an
Engineering discipline, and the State ofNJ requires firms providing such services
to hold a valid Certificate of Authorization.
y Significant construction experience
y Excellent communication abilities to keep all parties properly informed
, Familiarity with New Jersey Local Public Contracts Law
y
'\ Ability to effectively interface with the Design Consultant and other Project

Team firms
);;- Proven ability to face and resolve conflicts and problems
CCMS possesses all of the above attributes, and the team is well qualified to provide the
services of this Proposal.
During the construction period, all correspondence, communication and submittals
associated with the Project and Inspection Management Team will be forwarded and/or
copied to the Design Consultant, Owner and others as appropriate. We will work closely
with the Design Consultants - they are a critical element in the success of the Project.
Our objective, and the basis of our services, will be to have the project completed on
-- time, in a safe manner, in accordance with the requirements of the project's plans and
specifications.

Scope of Services
The Program Management services provided by Consolidated Construction Management
Services, Corp. (CCMS Corp.) are presented in this section.
Desi!!n, Bid and Award
1) Interface with the Architect and other design team members to develop plans and specifications for the
improvements.
- 2) Manage the Bid advertisements and distribution of documents to prospective Bidders.
3) Receive inquiries and questions from prospective Bidders and provide response as appropriate.
4) If and as required, prepare addenda to the Contract Documents and distribute as required. Technical
information for the addenda will be developed and provided by the design consultants.
5) Attend the Bid Opening, and other meetings regarding Contract Award.
6) Schedule, contact attendees, and conduct the pre-construction conference( s), and issue meeting
minutes to same.
7) Schedule and attend other technical/quality control special meetings between Contractors and design
consultants or others (i.e. Utility coordination meeting, quality control procedures review meeting).
Construction Period
1) Provide full-time, on-site personnel during the active construction period to inspect the work and
provide the services listed herein. Schedule, coordinate, and direct as the Owner's representative, the
work of multiple prime contractors for the renovation of the Boro of Carteret Municipal Building.
- 2) Schedule and conduct regular progress meetings with the contractor(s), and prepare and distribute
minutes accordingly. Representatives of the design consultants and others shall also be in attendance.
3) Monitor progress of the contractor(s) against their Contract schedule. Require schedule updates from
Contractor as deemed necessary.
4) Receive Request for Information (RFI) from Contractors, coordinate with design consultants as needed
and respond accordingly.

5) Manage the submissions from the Contractor (administrative submittals, shop drawings, samples etc)
to the reviewing party and then the return to the Contractor. In conjunction with the Design Consultant,
review and approve Contractor submittals. Engineering submissions shall generally be reviewed and
approved first by the Design Consultant, and other submittals (i,e" construction/operations submittals)
shall be reviewed and approved by CCMS. Monitor the status of submission and review of shop
drawings and other technical submittals.
6) Receive and review Contractor(s) monthly Payment Applications, review against work in place, review
with design consultants, negotiate with Contractor as needed, and make recommendations for Payment
to the Boro of Carteret.

7) Observe and inspect the construction work at the site during all phases of construction. Document such
inspection with the use of daily reports for the project. Provide copies to the Bow for their files.
8) Schedule and/or monitor inspections by others (i.e., municipal building code inspections, State
inspections). Also schedule and coordinate the digging of test pits for utility location and verification.
Excavation work to be by the Contractor.
9) Review submission of Contractors safety program and comment where applicable.
10) Coordinate delivery, installation and follow-up services for Owner furnished equipment (if applicable).
11) Perform and/or manage, monitor and coordinate quality control testing and inspections by others. This
includes preparation of concrete cylinders, compaction density testing, and the reports for these and
other inspections. We will utilize an independent testing finn to perfonn this work. In accordance with
the RFP, there is an allowance in our Contract Proposal to cove this work. The specific items to be
tested include structural concrete, compaction density, materials investigations (i.e., sieve analysis),
asphalt field testing, and asphalt plant inspection.
12) Provide notifications to Contractor of deviations from Contract Documents noted by field inspections
and follow-up as needed if such deviations are not corrected.

13) Identify problems encountered in accomplishing the work, review with the design consultants and
recommend appropriate actions to the Owner to resolve these problems with a minimal impact on
project budget and schedule,
14) Maintain project files, including daily reports, correspondence, pay application, change-order and
claims files.
15) Receive, log, distribute and respond to Contractor communications.

16) Coordinate, III concert with the design consultants, with agencIes having jurisdiction.
17) Prepare and distribute written monthly reports of the construction activities, progress, delays, problems
and recommendations including a forecast of the activities anticipated for the subsequent month of
construction.
18) Coordinate issues and communications regarding extra work and time extension requests from the
contractors and make appropriate recommendation to the owner.
- 19) Coordinate and manage the necessary changes to the Contract Documents required due to authorized
change orders or to resolve issues in the field. Changes to plans and Specifications to be prepared by
the design professional.
20) Provide, coordinate and manage field survey work as required to verify the proper work of the
Contractor. Field survey party to be under the direct supervision of a licensed Professional Land
Surveyor.
21) Provide Value Engineering review for Contractor initiated proposals.
Construction Completion and Post Construction Phase
1) Coordinate and participate in final inspections and acceptance by all parties having jurisdiction over
the project.
2) Develop and distribute punch lists, and monitor Contractor corrective actions.
3) Make recommendations for release of final monies to Contractor(s).
4) In conjunction with the Design Consultant, manage and coordinate the preparation of final as-built
documents for the project.
5) Gather, organize and turn over to Owner the project documentation to be held by them.
6) Perform final close-out administrative tasks for the Project.

It is essentia1 for any project that the Project Team has the expertise and experience to
successfully implement and complete the Project. Our proposed Project team is a group
of professionals with a proven track record for many similar projects. Resumes are
enclosed, and we highlight the following:
.:. Project Executive - Lino A. DeAlmeida - Mr. DeAlmeida has over 30 years of
experience in the construction and engineering business. He is the founder,
president and Chief Executive Officer ofCCMS, and his vast experience and
knowledge are invaluable assets to our clients. Mr. DeAlmeida has constructed
projects throughout the world, and his experience includes public and private
sector jobs of all sizes. Mr. DeAlmeida is an attorney, and has served as National
President of the National Utility Contractors Association (NUCA), and President
of the NJ Utility Contractors Association (UTCA), two of the leading trade
groups of the construction profession.
.:. Project Manager - Dave Coats - Mr. Coats has over 25 years of experience in the
design and construction of public and private infrastructure projects. His
experience includes work as a Consulting Engineer, Contractor and Construction
Manager, giving him a full understanding of the critical relationship between
these disciplines. His projects include the $IB Great Plains Coal Gasification
Project, numerous public and private civil engineering projects, sophisticated
instrumentation systems and public safety radio systems. Mr. Coats is a licensed
Professional Engineer in the states ofNJ and P A.
.:. Construction Manager - Fred Rizk - Mr. Rizk has more than 30 years of
experience in construction project management and hands on field supervision of
projects throughout the United States. His projects included the demolitions,
~ augments, expansions and new construction of telecommunication, commercial,
residential and retail sites. The projects ranged in size from 100 sq. ft. to 100,000
sq. ft. and cost from $160,000 to 3 million. His responsibilities included
perfonning site surveys, directing the bid process, awarding contracts, interfacing
with landlords, contractors and architects. He also successfully monitored
contractor performance and managed work forces to complete projects on time
and within budget. Mr. Rizk is a certified Telcordia Installation Control engineer
and trained in OSHA, EH&S and Disaster recovery/risk assessment management.
- .:. Home Office support - Mike Krak - Mr. Krak has almost 40 years of experience
in the inspection and management of construction projects throughout the State of
New Jersey. His projects include road and bridge projects, parks and recreational
facilities, commercial buildings, and residential projects. He has performed a
significant amount of work for the NJ Department of Transportation and County
and local agencies throughout the state. Mr. Krak is a Certified Engineering
Technician.
.:. Home Office Support - John Schleer - Mr. Schleer has over 30 years experience
in the construction business and specific experience with over a dozen school
projects. John's resume includes supervision ofN. Plainfield schools, E.
Brunswick High School where he was a Construction Project Manager. Overall,
his site awareness is unequaled. Past projects include Westbury College, Old
Westbury, NY a $25M student center and administration building. In addition, a
total renovation of the Davis & Elkins College, Halliehurst Center $12M total
renovation of this historical building. His expert planning and coordinated efforts
have successfully played a key role in completion of countless construction
projects in his career.
.:. Home Office Support - Ron Zucker - Mr. Zucker has over 20 years experience in
the construction administration for numerous projects throughout the State. His
projects include facilities at Gateway National recreation Area, restoration of
historic structures, including the D&R canal, and a Child Development Center for
the United States Coast Guard. Mr. Zucker expertise includes scheduling, cost
engineering, and purchasing, and safety planning.

These individuals and subconsultants have proven track records for getting projects
successfully completed. They are all highly experienced and motivated and dedicated to
providing the citizens Union County with a successful project.

Basis of Compensation
- 1. For servíces beyond the scope of this Proposal, compensation to be based on enclosed hourly rates.
2, If services are not complete by December 31, 2004, enclosed rates to be adjusted to reflect increased
costs.
3, CCMS Corp. to provide home office personnel to perform services as outlined in the Scope of Services,
and part-time on site inspections during period of critical construction activity.
4, Temporary facihtíes on site, if and as needed, to be provided by others.
5, Subconsultant cost to be billed at actual cost plus handling fee.
6, Payment Schedule to be as per Agreement
Comments and Exclusions
I) Project expenses to include: fees by government authorities or utilities, photography or video costs,
reproduction costs, travel expenses, delivery be reimbursed at cost, plus 10% handling charge.
2) Coverage for overtime work by Contractor not included - to be provided as an Additional Service.
3) Costs for security or night watchman by others.
