A RESOLUTION OF THE CITY COUNCIL OF
THE CITY OF LYNWOOD TO APPROVE A
CONTRACT ADDENDUM FOR ALL CDBG FUNDED •
PUBLIC SERVICE PROJECTS
WHEREAS, during the period of April 19, 1986 to May 7, 1986,
HUD conducted a comprehensive monitoring review of the City of
Lynwood's CDBG program; and
WHEREAS, said review included two public service projects
implemented by subgrantees under contract to the City; and
WHREEAS, as a result of this review HUD recommends that the
City incorporate an addendum to all public service contracts
concerning program income, audits and tracking of program benefi-
ciary data; and
WHEREAS, the City is acting in compliance with said re-
commendations.

NOW, THEREFORE, the City Council of the City of Lynwood
does hereby determine, order and resolve as follows:
SECTION 1. Approve and adopt the attached contract addendum
for incorporation in all CDBG funded public service projects;
SECTION 2. Authorize the Mayor to execute the attached
public service contract addendum.
PASSED, APPROVED and ADOPTED this 5th day of August 1986.

Prooram Income
All program income derived from the use of CDBG funds shall beI
documented and reported to the HCDA Coordinator or designee of
Lynwood on a quarterly basis.
Audits
St. Francis b4edical Center is responsibile for conducting annual
audits of the Child Abuse Prevention program for Federal and fis-
cal compliance. Said audits must comform to single audit requre-
ments and are subject to HUD and City review.
Beneficiary Data
shall submit copies of household iniome
certification statements relative to client benefit on a quarterly
basis to the HCDA Coordinator or designee. Said statement shall
be submitted on all clients serviced. In the event this informa-
tion is not able to be ascertained, a written statement designating
the reasons for such shall also be submitted. Failure to provide
said reports will result in jeorpardizing future payment requests.
