REQUEST:
Please provide a copy of the insurance policy (and all endorsements to said insurance policy
or related to said insurance policy) which was issued to the law firm of Sweetapple, Broeker
and Varkas and/or Robert Sweetapple and which was in effect during January, 2015. This
would be the same insurance policy which had a $25,000 deductible and which Robert Sweetapple
requested that the own o u Cream pay the $25,000 deductible on behalf of 1m and /or
his firm, as discussed and as approved at the Town Commission meeting held on January 9, 2015.

THIS REQUEST IS MADE PURSUANT TO ARTICLE 1, SECTION 24 OF THE FLORIDA CONSTITUTION AND CHAPTER 119,
FLORIDASTATUTES
