FOR THE WEDGEWOOD DRIVE CATCH BASIN REPLACEMENT WHEREAS, the Borough of Carteret previously awarded an Emergency Repair Services Contract; and -,-.. WHEREAS, pursuant to said contract the Borough Engineer, John DuPont, requested B & W to replace a catch basin on Wedgewood Drive; and WHEREAS, B & W Construction submitted an invoice for the above described work, and the catch basin replacement and the invoice for the same have been inspected and approved by Mr. DuPont, who has recommended payment of said invoice; and WHEREAS, the chi~f financial officer has certified that funds are or will be available for said purpose. NOW THEREFORE BE IT RESOLVED by the Mayor and Council of the Borough of Carteret as follows: l.That the attached invoice from B & W Construction for the Wedgewood Drive Catch Basin Replacement in the amount of $7,446.60 is hereby approved for payment pursuant to the recommendation of Borough Engineer, John DuPont. 2.That a copy of this Resolution be forwarded to the Borough's Purchasing Department and the Borough Engineer's Office. In order for the Engineer to issue a Notice to Proceed, these additional conditionslrequirements must be complied with: 4. The Owner or Owner's Counsel, must approve the executed documents, bonds and insurance certificates delivered by you, 5. You must attend a preconstruction conference. The preconstruction conference schedule will be forwarded under separate cover. 6. You must submit and obtain the Engineer's approval of the Progress Schedule required by Section GC.24,3, 7, You must submit and obtain the Engineer's approval of a submission schedule for the Shop & Setting Drawings required by Section GC.23. 8. You must advise the Engineer in writing of the following: 8.1- Name, address, business & 24 hour emergency phone number of Superintendent 8.2- Name, address, business & 24 hour emergency phone number of Responsible Person - Site Safety 9. In accordance with Section GC.21.5, you must submit a list of known Subcontractors (in addition to any listed with your proposal pursuant to NJSA 40A: 11-16) with a description of the work to be subcontracted, addresses and phone numbers, Similar information must be submitted as additional subcontractors are identified. Evidence of registration pursuant to NJSA 34: 11-56-48 et. seq. must be submitted for all subcontractors prior to starting work.
