Public Comment- Members of the community are invited to share ideas, state opinions or ask questions Mary Ellen Gambon from the Hopkinton Independent joined the meeting virtually at 5:28 p.m. Request to Deputize a New Metrowest Tobacco Agent for the Town of Hopkinton Ms. LaRoche requested the deputization of the new Metrowest Tobacco Agent Parivallal Thillaigovindan for the Town of Hopkinton. Ms. Miloslaysky motioned to deputize Parivallal Thillaigovindan as the Tobacco Agent for the Town of Hopkinton. Vice Chairman Mannan seconded. Request to Approve the Meeting Minutes of September 13 , 2023 Ms. LaRoche requested approval of the Board's meeting minutes of September 13, 2023. Request to Reimburse the Director Ms. LaRoche requested the following reimbursements to be approved for the Director. $67.55 Registered Sanitarian Renewal $74.68 Mileage/Yankee Conference $184.49 Yankee Conference $326.72 BOH October 16, 2023 -MEETING Minutes Page 1 Ms. Miloslaysky motioned to approve the Director's reimbursements, as requested and detailed by Ms. LaRoche, in the total amount of $326.72. Records Digitization Update/IT Manager Ms. LaRoche stated that on September 28, 2023 the IT Manager, Mr. McAuliffe, Chairman Jacobs and herself were shown a beta testing example of how the property files would show and be organized in the new scanning software, laserfiche. The example was presented to the satisfaction of the health department and was very easy to follow. Ms. LaRoche said that she is very pleased and excited to see this project moving forward and also stated that in the very near future, according to the IT Manager, the scanning of files will begin. Mr. McAuliffe noted that IT is waiting for a large scanner to be delivered to accommodate the larger size design plans. Department's FY 2024 September Budget Expense Summary Ms. LaRoche offered the department's September 2023 budget expense summary and stated that for the month of September the department's expenditures were a total of $1. 999.81 which leaves a balance in the operating budget of $70,100.15 to be expended by June 30, 2024. Strategic Plan Update Mr. McAuliffe noted that the representatives of the Hopkinton South Asian Circle and the Chamber of Commerce slots have been secured. The first meeting with the Advisory Committee members will take place on Tuesday, October 24, 2023 and believes that the first survey from Polco will be issued to the advisors at that meeting. He voiced that the project is moving forward very well. Chairman Jacobs asked if any board member(s) are invited to the meeting. Mr. McAuliffe stated that board members are welcome to attend and will email the link to them. PFAS Update Vice Chairman Mannan voiced her understanding that the department is extremely frustrated with the Department of Environmental Protection (DEP) not giving nor having direct and clear guidance for the public and local boards of health in regards to the PFAS standard requirements. Mr. McAuliffe notified the Board that he has communicated with the DEP and the Environmental Protection Agency (EPA) on how they are or will be proceeding to justify and educate the public the difference in two different PFAS reporting requirements. The public drinking water standard is 4 ppt and the requirement for private drinking wells is 20 ppt. He has not heard back from either of the departments as of yet in regards to his question. He noted that what the DEP has done, which is the very first time that DEP has acknowledged this, is that PFAS is in everything. Mr. McAuliffe informed the Board that DEP is BOH October 16, 2023 -MEETING Minutes Page 2 reiterating the health department's research data that was completed two years ago by the department's intern in regards to what PFAS is and where it comes from. He said that Hopkinton's PFAS concentration is low (below 20 ppt) compared to municipal water supplies. Mr. McAuliffe strongly noted that DEP has conflicting information on their website stating that if a private well PFAS testing is above 20 ppt your property will be considered a hazard waste area and in another section of their website it states that DEP will not consider the property a hazard waste site if the count is higher than 20 ppt. McAuliffe noted that filters in refrigerators and water dispensers are effective at removing PFAS and noted that about 20% of all PFAS comes from tap water. He strongly encouraged residents to contact their State Representatives to have these chemicals taken out of our food and manufacturer(s) suppliers. Chairman Jacobs asked when the PFAS problem would likely be resolved for municipal water supplies. Mr. McAuliffe said the problem would likely be resolved when the town connects to the MWRA. Mr. McAuliffe announced that any resident that wants to test their water well needs to utilize a Massachusetts Certified Laboratory. The DEP has a link to all of their certified laboratories on their website. CART Update Mr. McAuliffe notified the Board that the Department of Public Health (DPH) has published a dashboard which illustrates how the communities that are participating in a public health collaborative are performing based on the performance standards that DPH has created. After a further review, he noticed that the current collaborative is outperforming all the other collaboratives out of state. The only collaboratives that have outperformed the MetroWest Shared Collaborative, which the department is part of, are those communities that are part of the Barnstable Public Health Collaborative which has been in place for about 10 years and the Worcester Public Health Collaborative which has been disassembled and had established their policies and procedures for a number of years. Mr. McAuliffe noted that the hopkinton health department is at the top of the high performing ranking based on feedback from the DPH and noted that the towns of Hopkinton, Natick and Framinham have pulled our collaborative up (MetroWest Shared) . He noted that he has requested from the DPH liaison an individual town ranking metric; however, the liaison has not produced it as of yet. Vaccination Discussion Mr. McAuliffe and Nurse Carter informed the Board that the State is no longer supplying free Covid vaccines to towns. Due to that decision it is impossible for the Health Department to provide vaccines at $150 each to town residents due to the department's budget not being able to absorb the cost. Nurse Carter stated that there is a bridge program for children and those individuals who do not have medical coverage. Mr. McAuliffe voiced that the State still owes reimbursement to the department for more than 600 shots last year which impacts the department's budget. The Board voiced their understanding of the situation and thanked Mr. McAuliffe and Nurse Carter for their efforts. Public Health Nurse Report Public Health Nurse updated the Board on the following: Hopkinton Health Department Public Health Nursing Report 10.16.2023 Disease Surveillance New Dashboards rolled out per DPH—will reflect rates of RSV, influenza, and Covid. The Department had received several food and business complaints related to illegally operating apartment rentals (no heat or smoke detectors) and unpermitted businesses. The Department is working with the property owner and HOA management companies, homeowners and business ownership to resolve these issues. Each has been issued a cease and desist letter. Well and Septic - 1. The Director has met with the Emergency Management Group to discuss the discoloration of the Town's water. The discoloration is due to manganese and iron deposits that have collected in the piping and the base of the well. Arbovirus / Mosquito Control 1. 37 communities to the southwest of Hopkinton remain in a moderate to high risk category as a result of EEE positive mosquito pools being detected. Hopkinton remains in a low risk category. The mosquito risk will end after the first killing frost. BOH October 16, 2023 -MEETING Minutes Page 5 Allergen - 1. The Hopkinton Independent has interviewed Reese Robledo, her family and the Director about the playground allergen signage. Additional signage is planned for EMC park. The MDPH/APHC will assist the Health Department in taking this program statewide. MDPH Work - 1. The Health Department is due to receive COVID-19 Test Kits from the collaborative and the MDPH, at no cost. Additional kits have been requested for PJB. 2. The Director and Public Health Nurse will be meeting with the MDPH to discuss the challenges of operating a vaccination program given the necessity of purchasing Influenza, COVID-19 and RSV vaccines on the private market. 3. We are working with PJB to offset the loss of fresh fruit and vegetable deliveries from GBFB. 4. We submitted our annual Opioid Disbursement Report to the Commonwealth. 5. The Director will be working with UMass, Tufts and Tick Report staff to collect deer blood and deer ticks for disease. 6. Simone, Kelechi and I will be attending the MHOA conference next week. Even though these have just been picked up, we are already working with the vendor on how we can improve on the next 'round - namely, by Hopkinton doing a little more pre -work and also utilizing barcode labels to help streamline the process. A sub -component of that is we may start to scan files dated in the year of 2023 here internally, on premise. Additionally, I expect we'll start to have the hierarchy structure built out soon - and so can also talk about how new documents can be digitally ingested into the new system, and perhaps bypass even having to physically go into a manilla folder. I will continue to keep you all updated. Coincidentally, Nidia stopped by Fruit Street earlier today, and so is already aware that those 10 boxes have shipped out. ,If there is a need for any documents that have been shipped out, please send an email to both Lucas and I, and we will work with the vendor to have them digitize that folder as quickly as possible. While we have an agreed upon understanding of how we will file and tag the documents, we've been working with our laserfiche electronic document management system partners to ensure we have the proper settings, structure, and processes in place to support this. We have also been working with the scanning company to ensure the requirements are met, and to work through the logistics of the process. We are close to having the first "beta run" of boxes sent out, and I expect that will happen by the middle of November, if not sooner. I will continue to keep you updated on progress, and welcome any feedback or questions you have in the meantime. There are ten boxes total, and on the PDF I have sorted by address street name for ease of reference for you - though note that the box numbers they are in don't necessarily follow alphabetical order. The boxes are named HD1 through HD10. This will be important if you get a request for a file that is in one of these boxes, while it is offsite for scanning. If that happens, we can get in touch with Inception, and they'll scan that address within a day or two so that we can pass along the digital copy to the requestor. These boxes are still onsite in the IT Department building at Fruit St. I will let you know once they leave the premises and go to Inception. Additionally, IT has boxed up a few other addresses files, though we do not yet have the manifests for those. We will be testing out a different approach on some of those, with us possibly doing some of the doc prep here, and also scanning the 2023 files independently, here in-house - to get a feel for if this approach may result in a time or cost savings for the town. If you are looking for an address in the garage files and don't see it, and it's also not on the manifest list for files sent to Inception for scanning - we can show you where those other boxes are here in the office. The Department had received several food and business complaints related to improper temperature control of a food product, distribution of expired food, unpermitted food distribution and illegally operating apartment rentals (no heat or smoke detectors). The Director addressed the temperature control complaint, met with the Store Director and observed all cold holding to be compliant. The complaint against the food pantry was unfounded as they are permitted to distribute out of code (food beyond its use by date). A vendor was observed to be "delivering food" to the rear of the CVS Pharmacy. The Department is working to identify the source of the food that is being distributed. The housing case remains open. Inspectors from the municipality have been dispatched to the home to assess whether the repairs were completed without proper permits and inspections. The owner had been issued a cease and desist letter. Well and Septic - 1. The discharge from the Legacy Farms treatment plant was determined by the MDEP to be out of regulatory compliance. The MDEP issued a notice of non-compliance to the owner. The owner and operator have been ordered by the MDEP to bring the plant into compliance. Arbovirus / Mosquito Control 1. The Arbovirus season has ended. Last week's frost should have killed the majority of adult mosquitos. Based on the presence of EEE positive water bodies to the southwest of Hopkinton this Summer and Fall, the Department will be engaging the Central Massachusetts Mosquito Control Project to treat our swamps and water bodies with an organic larvacide next Spring. Allergen - 1. The Hopkinton Independent has interviewed Reese Robledo, her family and the Director about the playground allergen signage. Fox25 may interview her before December. The signage for EMC park has been approved. The MDPH/APHC will assist the Health Department in taking this program statewide. Our intern will be developing additional program content. MDPH Work - 1. The Health Department is distributing COVID-19 test kits again to the public at no cost. Additional kits have been delivered to PJB. �y. 2. The Department is auditing the MDPH's insurance claim processor (refer to vaccination discussion) 3. The Department started working with the school district to recover food from the free breakfast and lunch program. Recovered food has been delivered to PJB. 4. We resubmitted our annual Opioid Disbursement Report to the Commonwealth. Though Y&F Services and Health cannot access the settlement money until we go to town meeting, the town has received more than $45,000 to date. 5. The Director is working with UMass, Tufts and Tick Report staff to collect deer blood and deer ticks for disease surveillance.
