SUMMARY STATEMENT:
Attached is a Policy Amendment for Metropolitan Life Insurance company for dental insurance
services. The change reflects the addition of a $50 deductible and the increased benefit of no
cost coverage for preventative dental services.
RECOMMENDED ACTION:
Approve the Policy Amendment and authorize the Mayor to sign all necessary documents
effective October 1, 2005.
