HomeMy Public PortalAboutParks & Recreation Contracts for Services - Scottie MiltonParks & Recreation CONTRACT FOR SERVICES THIS CONTRACT FOR SERVICES made and entered into this t — day of the City of Crestview, a municipal corporation, hereinafter referred to as "City" and SC referred to as "Vendor". and between MILTON , hereafter WITNESSETH: WHEREAS, the City has chosen to procure services for the recreation of children; and WHEREAS, the City charges the Parks & Recreation Division with the overall responsibility of administering said services; and WHEREAS, the City directs that measures be taken to procure services of qualified providers through a contract for services not to exceed FY 2013-2014 Budget payable in one lump sum of $ .30.00 NOW, THEREFORE, in consideration of the mutual covenants contained herein the parties agree as follows: 1. VENDOR agrees to perform scheduled work on behalf of the CITY. 2. VENDOR agrees to receive Form 1099 at the end of the City's business year for the tax purposes. 3. VENDOR understands that managerial direction and supervision of assigned duties shall emanate from, the Parks & Recreation Division. 4. CITY agrees to provide facilities essential to carryout services by the VENDOR. 5. Provisions of this contract may be terminated at any time with thirty days notice to either party in written form. 6. This contract constitutes the entire agreement between the parties and any changes or amendments hereto must be reduced to writing and signed by both parties to be legally binding. For federal tax purposes, you are considered a U.S. person if you are: " An individual who is a U.S. citizen or U.S. resident alien, " A partnership, corporation, company, or association created or organized in the United States or under the laws of the United States, " An estate (other than a foreign estate), or " A domestic trust (as defined in Regulations section 301.7701-7). Special rules for partnerships. Partnerships that conduct a trade or business in the United States are generally required to pay a withholding tax on any foreign partners' share of income from such business. Further, in certain cases where a Form W-9 has not been received, a partnership is required to presume that a partner is a foreign person, and pay the withholding tax. Therefore, if you are a U.S. person that is a partner in a partnership conducting a trade or business in the United States, provide Form W-9 to the partnership to establish your U.S. status and avoid withholding on your share of partnership income.
