As a follow up to our conversation, attached is the lease documentation you requested. In order to process your lease
request without delay, please make sure you do the following:
1. Sign and date the signature page on the attached document to confirm your acceptance of the lease
agreement.
2. Please complete the document by writing in your Tax ID#.
3. Please include a copy of your current tax exemption certificate, if applicable.
4. Initial any changes you make to the name, title, address and contact information.
