ADDENDUM
THIS (Numb2 to Contract No. 2-2013) is made and entered into this
� day 7����014, by and between Richmond, Indiana, a municipal
corporation acting through its Board of Public Works and Safety with its office at 50 North 5th
Street, Richmond, Indiana, 47374 (hereinafter referred to as "City"), and Elite Clean, 543
Oakview Drive, Richmond, Indiana, 47374 (hereinafter referred to as "Contractor").
WHEREAS, City and Contractor entered into Contract No. 2-2013 on January 10, 2013, and
Addendum No. 1 to said Agreement on December 12, 2013; and
WHEREAS, City wishes to continue to retain Contractor to continue to provide floor cleanin
and maintenance services for the Richmond City Building, located at 50 North 5
Street, Richmond, Indiana, as set forth within the scope of Contract No. 2-2013.
NOW, THEREFORE, in consideration of the mutual promises and covenants herein contained,
including the above stated recitals, City and Contractor hereby agree as follows:
1. The City and Contractor executed and entered into Contract No. 2-2013, on
January 10, 2013 and Addendum No. 1 to said Agreement on December 12, 2013.
Pursuant to Contract No. 2-2013, Section IV Term of Agreement, the City has an
option to renew the Agreement for a third calendar year at a three percent (3%)
increased cost, which option is exercised pursuant to this Addendum. Contractor
shall continue to provide floor cleaning and maintenance services for the
Richmond City Building as identified within Contract No. 2-2013 for the 2015
calendar year. Such services shall be performed in a manner consistent with
Contract No. 2-2013, which is hereby incorporated by reference and made a part
hereof, at the increased cost amount that is not to exceed One Thousand One
Hundred Seven Dollars and Twenty-five Cents ($1,107.25) per month during
the 2015 calendar year.
2. City and Contractor expressly agree that all other terms, conditions, and
covenants contained in Contract No. 2-2013, shall be applicable to the work
performed pursuant to this Addendum, unless such term, condition or covenant
conflicts with this Addendum Number 2, in which case this Addendum Number 2
shall be controlling.

3. Both parties hereby agree that with the exception of the extension of the contract
date of termination pursuant to the exercise of the option to renew the contract for
a third year and with the exception of the three percent (3%) cost increase,
Contract No. 2-2013 shall remain intact and unchanged and in full force and
effect.
4. Both parties agree that any person executing this Contract in a representative
capacity hereby warrants that he or she has been duly authorized by his or her
principal to execute this Addendum.
IN WITNESS WHEREOF, this Addendum is execute and approved by the undersigned
representatives of the City and Contractor this 2J day of 2014, to be
effective in accordance with its terms as of January 1, 2015.
