the Town of Gulf Stream Town Hall on September 22, 2015 which are in possession of the Town of Gulf
Stream.

mean each of the following: the Town of Gulf Stream. its Commissioners. its Manager, its employees, its
Police Department, its Police Officers and its Counsel (including, without limitation, the following law
firms) Sweetapple. Broeker & Varkus; Richman Greer, PA: and Jones. Foster, Johnston & Stubbs.
(including, without limitation, the attorneys, employees and partners of each such law firm.)

ON THE ABOVE DATE AND TIME 1 WAS DISPATCHED TO THE GULF STREAM TOWN HALL LOCATED AT 100 SEA
RD, GULF STREAM, FL. REFERENCE A MEDICAL CALL. UPON ARRIVAL I WAS MET BY SGT. J. PASSEGGIATA
AND MEMBERS OF THE DELRAY BEACH FIRE DEPARTMENT. THE FIRE DEPARTMENT PARAMEDICS WERE IN
THE PROCESS OFTREATING THE VICTIM. THE VICTIM WAS LATER IDENTIFIED AS MR. MARTIN O'BOYLE.
MR O'BOYLE IS REPORTING A INJURY FROM A FALL. MR. O'BOYLE WAS TREATED AT THE SCENE AND
TRANSPORTED BY AMBULANCE TO BETHESDA HOSPITAL. REFER TO SGT. J. PASSEGGIATA INVESTIGATION
DOCUMENTED UNDER CASE #15-1672. END OF REPORT.
