THURSDAY, DECEMBER 18,2014; 5:00 PM
The Board of Public Works and Safety of the City of Richmond, Indiana, met at 5:00 P.M., Thursday,
December 18, 2014, in the Council Chambers at the Municipal Building, 50 North 5th Street, Richmond,
Indiana.
Tony Foster called the meeting to order and the following business was conducted:
ROLL CALL
Board members present were: Tony Foster and Dick Foore. Absent was Vicki Robinson.
Also in attendance were Kim Vessels, Assistant City Attorney; Monica Burns Deputy City Clerk; Police Lt.
Donnie Benedict; Elijah Welch, Engineer with the Richmond Sanitary District; Sue Roberson, Human
Resources and Media.

Mr. Foore moved to approve the December 11, 2014 minutes, supported by Mr. Foster. Upon a vote viva
voce, the motion carried. Mr. Foster moved to amend the minutes regarding the Conflict of Interest
Disclosure Statement to be reflected in the minutes that it was to be effective as of December 4, 2014
supported by Mr. Foore. Upon a vote viva voce, the motion carried.

Mr. Foore moved to approve the Claims for payment for December 12, 2014 supported by Mr. Foster.
Upon a vote viva voce, the motion carried. Mr. Foore moved to approve the Claims for payment for
December 19, 2014 supported by Mr. Foster. Upon a vote viva voce, the motion carried.
REQUESTS

Layne Heavy Civil is working on the East Side Interceptor Replacement Project Phase III
Mr. Foore moved to approve, supported by Mr. Foster. Upon a vote viva voce, the motion
carried.

CONTRACTS
• CONTRACT NO. 141 -2014 — ELAP SERVICES, LLC
To provide claim review and audit services for the City's insurance plan.
There will be compensation paid of 12% of payable charges subjected to the audit.
Mr. Foore moved to approve, supported by Mr. Foster. Upon a vote viva voce, the motion
carried. Mr. Foore moved to allow Sue Roberson to sign the contract, supported by Mr.
Foster. Upon a vote viva voce, the motion carried.
• CONTRACT NO. 142 -2014 — ELAP SERVICES, LLC
This is a HIPPA Business Associate Agreement for the City's insurance plan.
Mr. Foore moved to approve, supported by Mr. Foster. Upon a vote viva voce, the motion
carried. Mr. Foore moved to allow Sue Roberson to sign the contract, supported by Mr.
Foster. Upon a vote viva voce, the motion carried.
• CONTRACT NO. 145 -2014 —CUMMINS ELECTRIC, INC
To provide on -call general electrical repairs for the Richmond City Building for 2015
to be paid at a rate of $64.00 an hour.
Mr. Foore moved to approve, supported by Mr. Foster. Upon a vote viva voce, the motion
carried.
• CONTRACT NO. 146 -2014 — INDIANA VISION SERVICES, INC
To provide vision care provider benefits for the City of Richmond, Indiana.
Mr. Foore moved to approve, supported by Mr. Foster. Upon a vote viva voce, the motion
carried.
CHANGE ORDERS - None
ADDENDUMS
• CONTRACT NO. 86 -2014 ADDENDUM NO. 1 H.J. UMBAUGH ASSOCIATES
This is to extend the terms of the contract to February 28, 2015
Mr. Foore moved to approve, supported by Mr. Foster. Upon a vote viva voce, the motion
carried.
