Dear Custodian of Records,
This email is a singular request for a public record. Please respond to this public record request in a singular manner and do not combine this
public record request with any other public record requests when responding.
Before making this public record request, I first searched the public records portion of your agency's website hoping I could locate the public
record I seek without having to trouble you for it. I specifically searched the Building Deoartment Land File and the Architectural Review
Properties by Address. Unfortunately I can not find the records I wish to examine.
I request you provide for my inspection the public record which is:
The first 3 pages of the most recent permit for the removal of interior framing at 2546 Avenue Au Soleil as evidenced by the
two attached photos taken on October 21, 2015.
1 make this request pursuant to Article 1, Section 24 of the Florida Constitution and Chapter 119 of the Florida Statutes.
If you contend that any of the records I am seeking, or any portion thereof, are exempt from inspection or disclosure please cite the
specific exemption as required by 4'119.07(1)(e) of the Florida Statutes and state in writing and with particularity the basis for your
conclusions as required by 4 1 19.07(l)(f) of the Florida Statutes.
Please take note of 4119.070)(c) Florida Statues and your affirmative obligation to (1) promptly acknowledge receipt of this public
records request and (2) make a good faith effort which "includes making reasonable efforts to determine from other officers or
employees within the agency whether such a record exists and, ifso, the location at which the record can be accessed." I am,
therefore, requesting that you notify every individual in possession of records that may be responsive to this public records request to
preserve all such records on an immediate basis.
If the public records being sought are maintained by your agency in an electronic format please produce the records in the original
electronic format in which they were created or received. See &I 19.01(2)(f), Florida Statutes.
Please provide only those records for inspection that do not require extensive use of information technologies or extensive staff time
or both in excess of 15 minutes. Please take note of 41 19.070)(01(d) Florida Statues and if you anticipate that any records exist, the
production for inspection of which will require extensive use of information technologies or extensive staff time or both in excess of
15 minutes then please advise me of the cost you anticipate to be incurred by your agency prior to incurring this cost. Please do not
incur any costs on my behalf without first obtaining my written authorization to proceed.
If you anticipate the need to incur any costs that I would be statutorily required to pay in order to inspect these public records which
would exceed $1.00 please notify me in advance of your incurring that cost with a written estimate of the total cost. Please be sure to
itemize any estimates so as to indicate the total number of pages and/or records, as well as to distinguish the cost of labor and
materials. Again, please do not incur any costs on my behalf without first obtaining my written authorization to proceed.
I hereby reserve all rights granted to me under the Florida Constitution and Florida Statutes.

Dear Town Manager Thrasher,
I am writing you regarding what appears to be a subject you have expressed interest in before,
namely construction without a permit.
Forwarded herein is a public record request, with attached photos, I made to the Custodian of
Records for your agency on Oct 22, 2015. The request was for: The first 3 pages of the most
recent permit for the removal of interior framing at 2546 Avenue An Soleil as evidenced by the
two attached photos taken on October 21, 2015.
The Custodian replied by email today: Please be advised that no such records exist. That email
is attached for your reference.
The photos show a construction dumpster overfilled with what appears to be recently removed
wall framing. From the street one can also plainly see through the uncovered windows that
interior walls have apparently been removed. The dumpster is still on site as I write this in the
same condition this morning as it appears in the attached photos.
Perhaps the Custodian of Records is unaware of this permit. I am therefore asking you to confirm
with the Custodian that this permit exists and that I may inspect it. If the permit does not exist,
please take the appropriate action as mandated by the Town Charter and the Town Code of
Ordinances.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed and accepted by Miami -Dade County PERA -
Product Control Section to be used in Miami Dade County and other areas where allowed by the Authority
Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Section (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right
to have this product or material tested for quality assurance purposes. If this product or material fails to
perform in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may
immediately revoke, modify, or suspend the use of such product or material within their jurisdiction. PERA
reserves the right to revoke this acceptance, if it is determined by Miami -Dade County Product Control
Section that this product or material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building
Code, including the High Velocity Hurricane Zone.
DESCRIPTION: Series "PGT" Clipped Extruded Aluminum Tube Mullion - L.M.I.
APPROVAL DOCUMENT: Drawing No. 6300JR, titled "Impact -Resistant Aluminum Tube Mullions",
sheets 1 through 22 of 22, prepared by manufacturer, dated 08/29/11, signed and sealed by Anthony Lynn
Miller, P.E., bearing the Miami -Dade County Product Control Revision stamp with the Notice of Acceptance
number and expiration date by the Miami -Dade County Product Control Section.
MISSILE IMPACT RATING: Large and Small Missile Impact Resistant
LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state,
model/series, and following statement: "Miami -Dade County Product Control Approved", unless otherwise
noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been
no change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change
in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement
of any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure
to comply with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is
displayed, then it shall be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its
distributors and shall be available for inspection at thejob site at the request of the Building Official.
This NOA revises NOA 1110-0819.05 and consists of this page 1 and evidence page E-1, as well as approval
document mentioned above.
