Application form. As requested I have added a note at the bottom of the form describing the plans for tying into the 7th Street pavement. I also made mention of connecting to the water and sewer lines and running them through conduits in the bridge as discussed at our meeting in January. 2. An application fee check in the amount of$55. 3. Two sets of drawings each containing a plan view and a cross section of the proposed bridge. I have added a rebar plan description to the cross section drawing. Other than this one addition, the drawings are the same as approved by DNR and the City of Tybee earlier this year. 4. Copy of the DNR permit and placard dated March 12, 2007. DNR provided the City a full set of the permit documentation by copy of letter to me dated March 20, 2007. 5. The sealed drawings are attached. I asked the engineer, a former structural engineering work associate of mine, to certify the design for an H-20 loading. This is the typical design loading for a bridge of this nature and includes the load of a typical concrete truck. I obtained the following description of this load of an internet site: "H20 loading refers to a maximum front axle load of 8,000 pounds. Rear axles are limited to a maximum axle load of 24,000 lbs. for a single back axle and 16,000 lbs. for 2 rear axles spaced no more than 4 ft. apart. (See AASHTO document "Highway Bridge Loading", #3.8.2.2, page 20.). H2O axle loading converts to a tandem wheel load for 24,000 lb., single axles of 12,000 lbs. and a tandem wheel load of 8,000 lbs. for 2-16,000 lb. axles (as seen on tractor trailers) spaced 4 ft. apart." COASTAL MARSHLANDS PROTECTION ACT STANDARD CONDITIONS: 1. The project must comply, as applicable, for areas permitted herein, with all other federal, state, and local statutes, ordinances, and regulations and the applicant must obtain all licenses and permits prior to commencement of construction. 2. This permit does not resolve actual or potential disputes regarding ownership of,rights in or over the property upon which the subject project is proposed, and shall not be construed as recognizing or denying any such rights or interests. 3. All plans, documents and materials contained in this permit application, required by Coastal Marshlands Protection Act of 1970, as amended O.C.G.A. § 12-5-280 et seq. are a part of this permit and conformance to such plans, documents, and materials are a condition of this permit. No change or deviation from these plans, documents, and materials shall be permitted without prior notification and approval by the Department or Committee. 4. No further encroachment or construction shall take place within state jurisdiction, except as permitted by the Coastal Marshlands Protection Committee. Any modification of the plans or structure in the jurisdictional area must be reviewed and approved by the Department or the CMPC, as necessary,prior to construction. 5. The exact location and configuration of this project must be reviewed onsite and approved by Department staff immediately prior to beginning construction. Minor changes to the location Georgia Department of Natural Resources • Coastal Resources Division may be allowed or required in areas that have eroded or accreted subsequent to the original jurisdictional determination. 6. Adequate sedimentation control barriers must be properly installed prior to land disturbance to prevent silting and sedimentation from impacting jurisdictional marshlands. 7. No construction materials or debris may be placed, disposed of, or stored in jurisdictional marshlands. 8. A construction placard will be required to be obtained from the Department up to 30 days prior to the start of project construction and must be posted on site. This placard will include certain steps in the construction of the permitted project that must be approved by the Department prior to construction. 9. The permit must be posted onsite within twenty-four(24) hours of beginning construction. 10. Any incidental impacts associated with construction of this project must be rectified by restoring areas to their pre-construction topographic and vegetative states. 11. If the permitted improvements are damaged, fall into disrepair, become dilapidated or are not meeting its expected usefulness, it is not maintained at a serviceable level, and it is the responsibility of the owner to remove the improvements. A new permit will be required to retain and repair the structure, improvement or asset if it loses its structural integrity and is no longer serviceable. 12. The Coastal Marshlands Protection Committee is not bound in the future to protect any improvement or asset authorized by the permit. 13. A copy of these and all permit conditions must be supplied to the person in charge of construction. All contractors and subcontractors are responsible for strict adherence to all permit conditions. SPECIAL CONDITIONS: 1. Permittee shall provide a post-construction survey to the Georgia Department of Natural Resources, Coastal Resources Division within 30 days of completion of the permitted activity that locates the facility as indicated in the application materials. Such survey shall comply with the Georgia Plat Act O.C.G.A. 15-6-67 et seq. 2. Permittee shall notify the Georgia Department of Natural Resources, Coastal Resources Division within 30 days of completion of the permitted activity so that a compliance inspection can be conducted by CRD staff 3. Applicant must remove any buildup of debris at the location of the proposed bridge so as to prevent the restriction of stormwater flow below the proposed bridge. 4. When I have used the details I have selected the appropriate detail and just used a shorter wall at the same design as the higher one in the detail. Overdesign is rarely challenged. These details are for precast or cast in place. In talking with Mark Boswell, he advised he could provide additional services should you need them or could assist you with basic services if you were your own structural engineer. Therefore, I am copying Mark. In using details such as these to culverts applied to structures of lesser dimensions I do not recall being required to provide engineering analysis. However, modifying the base to allow for an open bottom would probably require additional analysis. I cannot find a link for a culvert with not bottom as you will need.
