Attached is a Task Order for Miscellaneous Traffic Reviews by our Consultant, Atkins. This is
an updated Task Order to provide work as needed under the new Continuing Services
Agreement. Wayne has reviewed it and is okay with it.

CONSULTING SERVICE AUTHORIZATION (CSA)
Service Authorization Number 2018-2
Project (Location): On -Call Traffic Concurrency Reviews/Minor Traffic Studies (Crestview, Florida)
This Service Authorization, when executed, shall be incorporated in and shall become an integral part of
the original Agreement for Consulting Services dated 3/14/2018 between the City of Crestview (CITY)
and Atkins (CONSULTANT).
I. Project Description: On -Call Traffic Concurrency Reviews/Minor Traffic Studies
II. Scope of Services:
Miscellaneous traffic engineering evaluation, reviews or minor studies to support the City of
Crestview in evaluating new development traffic impacts. Task assignments will be on an as
needed basis by City staff with no guarantees that any tasks will be assigned. Consultant will
provide an hourly estimate of work by task assigned with approval by City staff prior to initiating
work on task.
III. Items to be furnished by CITY at no expense to the CONSULTANT:
Traffic reports, traffic counts and supporting schematics associated with various developments
being proposed within the City of Crestview needing additional traffic analysis.
IV. Time of Performance
Each task assignment will be completed within 30 days after receipt of Notice to Proceed,
except for delays beyond the reasonable control of the CONSULTANT. The above noted items
of performance are not, however, a warranty or guarantee that the noted services will be
completed within such period.
V. Fees to be paid
For the Scope of Services as described, the CONSULTANT shall be paid on a limiting amount
basis, per task, using hourly rates based on the pre -approved master rate schedule (Attachment A)
with a total not to exceed contract amount of $7,500.00 for professional services.
