Box 213 Crestview FL. 32536 850-682-2919 alwaysbloominglandscape.com CUSTOMER City Of Crestview 121 Wilson Street N Crestview FL. 32536 Proposal/ Contract CUSTOMER NO. 507 PHONE 850-682-6132 ex 1 PROPOSAU CONTRACT NO. 507-103117 PROPOSAU CONTRACT DATE 10/31/2017 Proposal/contract Always Blooming Landscape Services proposes to provide material,labor,and equipment to provide the following: Description Qty Unit Price Amount Plantings 1 $1,750.00 / Per Year $1,750.00 Yearly plantings and maintenance for proposed new landscape: 1: Mow new turf areas in front of City Hall every other week, April- October and Once per month November- March 1:Install 150 mixed annual flowers spring and fall 2: Install 20 large bales of pine straw spring and fall 3: Cut back as needed Iris and any other plants in bed areas that require pruning mid to late fall 4: Once per month, "dead head" all annuals and weed beds as needed, apply nec. weed killer to help control unwanted growth Total yearly cost: $1750.00 Payable in 12 average monthly payments of $145.83 per month Total $1,750.00 Guarantee and payment terms Always Blooming Landscape Services guarantees all labor and materials to be free from defects for one year from date of completion. Payment terms: 50% deposit 50 % due in full day of completion Maintenance contract accounts are to be paid in full by the first of each month, ABLS reserves the right to terminate services at our discretion until account is paid and current. Please note that Palm trees have no guarantee from our suppliers or from ABLS. Sod Guarantee: We work closely with our supplier to ensure that sod is cut and deliverd in a healthy enviroment and state. Due to many circumstances out of our control such as watering, insects migrating from adjacent properties,fungus and improper use of fertilizers,we can only guarantee that sod will be healthy when delivered and installed.We will do our best to educate our customers as to proper care of their new turf. As required by Iaw,ABLS contacts utility locating services before tilling or digging.We will run irrigation and mark heads,but will not be held responsible for broken utility lines and or irrigation lines. Approved: Department Head Employee Signature FIN Rev 06/03 COPY This is Task Order No. 1 , consisting of 3 pages Task Order Format In accordance with the Crestview Community Re -Development Agency (CRA) Continuing Services Contract between Owner and Consultant for Professional Services dated March 12, 2018 ("Agreement'), Owner and Consultant agree as follows: Specific Project Data A. Title: Gateway Entrance Signage with Lighting B. Description: The project consists of the design of three distinct signages for entrances to the Main Street area of the City of Crestview for the Community Redevelopment Agency (CRA). Services will also include the preparation of construction and bidding documents for the project, as well as assistance with the bidding process, contract administration and resident project representative (RPR) services as requested by the CRA. 1. Services of Consultant Services include Engineering, Surveying (if required) and Architecture. Scope of Service shall include the following: Phase 1: Preliminary Engineering and Architecture Design 1. Meet with CRA officials to further define expectations for the three signage locations in more detail. 2. Preliminary concept plans/drawings prepared for CRA review. Phase 2: Design 1. Survey areas of signage (if needed) by Poly survey staff. 2. Complete design based on CRA staff input on preliminary design. 3. Prepare bidding and construction documents for the project to facilitate the CRA publicly bidding the signage project. 4. Additional meeting(s) as needed with CRA staff to finalize design. 5. Geotechnical survey (if needed) to be performed by others as sub -consultant Phase 3: Bidding Services 1. Provide complete bidding services for the project if requested by the CRA, or provide more limited assistance, based on CRA needs. 2. The final product will be a complete set of construction documents ready for bidding the signage project at three locations entering the Main Street Area. Phase 4: Construction Phase Services 1. Assist CRA personnel with contract administration from contract execution to contract closeout as needed. Also provide RPR services to monitor and report on field work as project progresses. Page 1 of 3 2. Owner's Responsibilities Section 3 of Agreement. 3. Times for Rendering Services Engineering/Architecture services to begin within 45 days of the Task Order date. 4. Payment for Services and Conditions A. Method of Payment, Standard Hourly Rates 1. The Standard Hourly Rates shall be as shown in Exhibit "B" of the Agreement. 2. The total compensation for services identified under this Task Order is estimated to be $22,040.00 based on the Engineer's judgement as to the effort required. Budget is based on Sr. Project Manager (24 hours), Architect (60 hours), CADD Tech (96 hours) and Structural Engineers (-$5,000), all estimates. 3. Budget ceiling for the Task Order shall be not to exceed $25,000.00 for Poly's services without prior authorization of the CRA including Geotechnical services, if required (see #5 Consultants below). 4. Payment for services under this Task Order shall be requested as the services are rendered and invoices shall be submitted no more frequently than monthly. B. Conditions None. 5. Consultants: Structural Engineer (preliminary- Westerra Development of Crestview) included in base proposal. Geotechnical Engineer (TBD) if required. Estimated cost for this service is $3,000. 6. Other Modifications to Master Agreement: None 7. Attachments: None 8.
