PEI-OMIT/RESOLUTION FOR PUBLIC UTILITY INSTALLATION
IN CITY RIGHT OF WAY
WHEREAS ��f,!/ . �; '�.T �,.. t-cvc-C. % e , a public
utility licensed to do business in the City
Medina, has made application
to the City for the installation of LDZC-
located at 17/(o� /
within the City of Medina;
NOW,. THEREFORE, BE IT RESOLVED that said application be granted
, in accordance with the plans and specifications submitted, subject to the
following conditions:
1. Street and Right of Way shall be restored to their
original condition upon completition of Installation
and Applicant shall be liable for settlement or other
damage caused by said installation for a period of
one year following completion.
