CITY OF MEDINA
RESOLUTION NO. 79-_5 1
PERMIT/RESOLUTION FOR PUBLIC UTILITY INSTALLATION
IN CITY RIGHT OF WAY
WHEREAS Continental Telephone Co. of MN, Inc. , a public
utility licensed to do business in the City of Medina, has made application
to the City for the installation of a telephone cable
located at 146' west of Cedar Avenue on Fern Street
within the City of Medina;
NOW, THEREFORE, BE IT RESOLVED that said application be granted
in accordance with the plans and specifications submitted, subject to the
following conditions:
1. Street and Right of Way shall be restored to their
original condition upon completition of Installation
and Applicant shall be liable for settlement or other
damage caused by said installation for a period of
one year following completion.
2. Completion Date: 10-15-79
3. Performance Bond in the Amount of:
4. Depth of Conduit: 36" minimum
