PERMIT/ RESOLUTION FOR PUBLIC UTILITY INSTALLATION
IN CITY RIGHT OF WAY
WHEREAS '

utility licensed to do business in the City of Medina, hi.s made application
to the City for the installation of 4a/)&__Ii/',d

within the City of Medina;
NOW„ THEREFORE, BE IT RIi:SOLVED that said application be granted
in accordance with the plans and specifications submitted, subject to the
following conditions: `
1. Street and Right of Way shall be restored to their
original condition upon c.ompletition of Installation
and Applicant shall be liable for settlement or other
damage caused by said installation for a period of
one year following cdmpletitin.
Applicant shall notify city before commencing work so that
drain tile can be located on north side of road.
