IN C1TY RIGHT OF WAY
WHEREAS

utility licensed to do business in the City of Medina, teas made application
to the City for the installation of

NOW„ THEREFORE, 13E IT RESOLVED that said application be granted
in accordance with the plans and specifications submitted, subject to the
following conditions:
1. Street and Might of Way shall be restored to their
original condition upon completition of Installation
and Applicant shall be liable for settlement or other
damage caused by said installation for a period of
one year following; completion.
