The Georgia Environmental Protection Division administers the rules. In most cases, the rules require both the owner and the involved contractors to assure the portion of the building involved in the project is thoroughly inspected by an Accredited Asbestos Inspector for materials that contain asbestos; and the removal of the asbestos before renovation, wrecking, or demolition begins almost without exemption. Georgia EPD requires a completed demolition notification from be submitted 10 workings days in advance even if no asbestos is present in the building. The 2012 file has been closed. The $250 permit fee paid for the prior permit will be credited toward a 2013 demolition permit. Please state clearly on the application all proposed areas of demolition and/or submit a site plan with the application. The attached site plan with the "demolition legend" will work but if you are applying for demolition work in any additional areas, please amend it as needed. For example, the 2012 permit application stated it was to: "remove house off of the top of the bunker leaving the bunker intact." If that is to be part of the scope of the 2013 demolition permit, please add it to the site plan since that work was not accomplished under the 2012 permit. Prior to the demolition permit being issued, pages 1 and 2 of the attached Contractor Registration application will need to be completed and submitted to Sharon Shaver (sshaver @cityoftybee.org/ 912-472-5072) along with a copy of B & B Demolition's local business license. The fee is $20 and registration will be valid the remainder of this calendar year. This process will also apply to all other contractors for the Nikola project. The demolition permit will obviously not include installation of the stone veneer. Please use the same 2-page application to apply for a separate permit for that prior to the start of work. We are actively planning to immediately restart the demolition project and additions/ 1/15/2013 Page 2 of 2 alterations to the 20 Taylor Street resider . As far as the Demolition is concerned, tt, . is an active, issued Demolition Permit for the subject property on file. Shore Protection Act Permit#387 issued July 24,2008 authorized construction of a single-family residence and driveways in a permitted footprint.All standard and special conditions of SPA Permit#387 remain applicable and enforceable. The proposed stone veneer will be attached with mortar and will cover the exterior fort walls down to existing grade.No digging or excavation is proposed in the sand dunes.A 4' wide path through the dune vegetation is proposed to allow workers space to work and pedestrian access to the site. Cutting will be done with hand tools(machetes).Plywood will be placed in the trimmed area to allow workers to use wheel barrows and scaffolding will be placed in the cleared dune area adjacent to the fort to access the wall.No vehicles are proposed in the dune area. The Department authorizes the proposed one time trimming and temporary placement of plywood and scaffolding in the dune area for the placement of stone veneer on the existing exterior fort wall as depicted in the attached description and drawings.No construction is authorized in sand dunes.No vegetation is to be disturbed beyond the limits of the project during construction. Any incidental impacts to the dunes or vegetation must be restored to pre-project vegetated and topographic conditions,any backfilling must be beach quality and obtained from an upland source and not from the beach.The trimmed vegetation must be allowed to grow back naturally. Any incidental impacts associated with the construction of this project must be rectified by restoring areas to their pre-construction topographic and vegetative states. 9. The public shall maintain rights of ingress and egress on the foreshore beach area seaward of the ordinary high water mark. 10.If the permitted improvements are damaged,fall into disrepair,become dilapidated,are not meeting their expected usefulness, or are not maintained at a serviceable level,then it is the responsibility of the owner to remove the improvements. A new permit will be required to retain and repair the structure,improvement or asset if it loses its structural integrity and is no longer serviceable. 11.The Shore Protection Committee is not bound in the future to protect any asset or improvements authorized by the permit. 12. The permit must be posted onsite within twenty-four(24)hours of beginning construction. 13.A copy of the above conditions must be supplied to the person in charge of construction. SPECIAL CONDITIONS: 1. No construction is permitted seaward of the landward toe of the dune. The footprint shall be reduced so that the entire structure is behind the toe of the most landward dune as designated by DNR staff on the survey by Whitley Reynolds,dated March 19,2008. 2. The mowed area seaward of the toe of the most landward dune must be restored with native vegetation. 3. A landscape plan must be provided to the Department for approval prior to installation. Landscaping will consist of native species and shall not change the jurisdiction of the Shore Protection Act in the future. 4. Should the dunes shift landward,which is considered a natural function of the sand sharing system,every effort must be made to protect the dynamic area. 5. During construction,no materials or equipment can be stored in the dune area During construction, silt fence must be placed landward of the existing dune to clearly delineate the dune area where no impacts are permitted. 6. All proposed driveways must be constructed of pervious materials. 7. In order to minimize the disruption of nesting activity from artificial lighting from the subject parcel, the permit must comply with the attached conditions entitled,"SHORE PROTECTION ACT O.C.G.A. § 12-5-230 SPECIAL PERMIT CONDITIONS REGARDING ARTIFICIAL LIGHTING SPECIFIC TO SENSITIVE SEA TURTLE NESTING AREAS"when developing a lighting plan for the subject parcel, 8. Samples of suspect exterior stucco, interior caulk, interior stucco, sheetrock, sheetrock mud, exterior caulk,roof shingles and roof paper(felt) were obtained from this home in order to determine if any asbestos containing materials (ACM's) were present before limited demolition can begin. As directed, the samples collected on December 7, 2011 have been analyzed for the presence of asbestos. These samples were received, bagged and sealed, tagged and shipped overnight to EMC Labs, Inc. of Arizona, (NVLAP#101926-0) a nationally accredited US EPA laboratory for bulk asbestos analyses by polarized light microscopy(PLM). Upon analyses of the results, the collected suspect samples were found NOT to contain asbestos. Therefore, no specialized abatement of these individual, specific materials appears necessary at this time. Due to the nature and suspected age of paints within the home, a sample of each type and character were received, bagged and sealed, tagged and shipped overnight to EMC Labs, Inc. of Arizona, (NVLAP#101926-0) a nationally accredited US EPA laboratory for lead in paint chip samples (EPA SW-846, Method 7420) by weight by volume. This report documents technical data generated by the collection of paint chip sampling and analyses as submitted to and performed by EMC Labs, Inc., via SOP Method#L01/1 and EPA SW-846 Method 7420. All physical sampling and collection of paint chip samples were performed within general accordance to the EPA §745.227 (f) sampling protocol methodology. Lead Based Paint (LBP) is determined as positive when the percentage of lead by dry weight is equal to or greater than(1.0 mg/cm2) or(0.5 %, 5,000 ug/g or 5,000 ppm). Upon review of the laboratory analyses, please be aware that one (1) of the ten (10) paint samples from the home contains a detectable level for lead. However, this sample (number 4, Step Red Paint) as collected does not exceed the federal maximum contamination level (MCL) as established by the US EPA for the determination of a"positive" action level reading for lead based paint. Therefore, no specialized abatement of any of these individual, sampled paints should require specialized abatement procedures.
