to comply with the Terms and Conditions listed on the reverse side of this form.
BE IT FURTHER RESOLVED THAT a certified copy of this resolution shall be submitted to the
Utah State Agency for Surplus Property and that the same shall remain in full force and effect until
written notice to the contrary is submitted to the said Utah State Agency for Surplus Property.
