A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF LYNWOOD
~" `i APPROVING PARTICIPATION IN THE
~ WORKERS' COMPENSATION PROGRAM OF THE
INDEPENDENT CITIES RISK MANAGEMENT AUTHORITY
WHEREAS, the City of Lynwood, ("City"), is a~nember in good standing of the
Independent Cities Risl< Management Autllority ("Authonty"), and
WHEREAS, the Authority is establishing a Worlcers' Compensation Program
("Prograin") thiough which inember cihes, includiizg~ this City; may secure workers'
coinpensation coverage, and ,
WHEREAS, 'the Autharity offers various self-insured retentions (SIRs) and
coverage limits as d~etennined by its Goven~ing Board, and
WHEREAS, the C1ty has received aild reviewed a copy of the Program Bylaws,
Memorandum of Coverage, and other related documents, and j,
~ WHEREAS, i~ is expressly ~understood that a coinmitinent by a participant in the
Pi-ograin must be for a period of no less t11an three (3) full program years, and
WHEREAS; ~t appears to be in the best uiterest-of t11e City tlZat it participate in
sard Prograin
NOW, THEREFORE, BE IT RESOLVED.; that the City Council of the City of
Lynwood does hereby find, determine, and declare as follows
1 Tlie City hereby elects to ~oin and remain a member of the Prograin for a
, period of not less than three (3) filll program years, provided the minimuin
participation re~uirements in the Prograin are met.
_ 2 The City hereby requests that, commencing on July l, 2003, tlie City's
industrial ul~ury claims be covered by the Program
~ 3 The Crty approves tl~e Program. Bylaws and Memorandurn of Coverage and
other related docuinents
4~ The City's designee is hereby aitthorized asld directed to execute such
documents as inay be.required to bind tlie City to the tenns and covenants
of the Program
5 The City sliall maintain a Certificate of Consent to Self-Insure for worlcers'
coinpensation liabilities frorn fhe Director of Industrial Relations, State of
'' _ Califoizua, and s1~a11 immed~ately advise t11e Authonty if there is any
~~ revocation or change in said Cerhficate ,

5 TIZe Ctty C1erlt of the City is hereby directed to certify a copy of the resolution
and forward same proinptly, by mail, to the Authority at the followii~g
